Peachtree Christian Hospice is located at 3430 Duluth Park Lane. If an individual is interested in reviewing the governing documents, the conflict of interest policy or the Form 990, he can speak with the Director of Community Relations who is located at the Facility. She will contact the Co-Chairs for the BOD of PCH who will ensure the documents are provided to the requesting individual.

Form 990, Part VI, Line 15b: Compensation Review and Approval Process for Officers and Key Employees
As regards the salary for the two PCH Officers, who are also Co-Executive Directors, the Compensation Committee reviewed compensation ranges for individuals with other similar non-profit entities. In addition, the Committee also ensured the comparison was based upon similarly qualified individuals who performed similar job functions. The information obtained was then reviewed with PCH's outside legal counsel. The Committee's recommendation was then submitted to the PCH Board for approval. The approval for the recommendation was 100%. Please note the individuals with a conflict of interest, including the two Officers, were not a party to the Compensation Committee's process. In addition, these same individuals were not permitted to witness or participate in the BOD's approval of the Compensation Committee's salary recommendation.

Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of Conflicts
Section 11 of Peachtree Christian Hospice's ("PCH") Bylaws, as revised in October 2001, satisfies the requirements specified in the instructions to the Form 990 regarding whether an organization maintains "valid" Conflict of Interest Policy. In addition, PCH requires annually that each member of the Board of Directors ("BOD") formally attest, via a signed statement, that they understand PCH's Conflict of Interest Policy. (A copy of Section 11 was provided to each BOD member in 2008). In addition, each member of the board is required to attest in writing, if they are aware of any Conflict of Interest which occurred, and if so, to disclose the nature of the Conflict of Interest. Each BOD member attested that he understood PCH's policy and no Conflicts of Interest occurred in 2010.

A complete copy of the Form 990 was provided to every member of the Governing Body (i.e. Board of Director's) prior to filing with the IRS. The Form 990 was reviewed in detail by the Co-Chairs of the Board and the Treasurer. Every other Board member was asked to attest, in writing, as to the level of review they performed. These attest statements are maintained by the PCH Treasurer.
