United Church Homes, Inc (UCH) is the managing agent for
United Church Residences of North Lewisburg, Ohio Inc.
Under the terms of the management agreement approved by HUD,
UCH is responsible for the operation of the organization.
Management fees and accounting fees are charged to the organization
on a per unit per month basis.

The Parent Organization, United Church Homes, regularly
and consistently monitors and enforces compliance with its
Conflict of Interest policy as follows:
1.The Organization's corporate By Laws require that all
members of the Board of directors (inclusive of
the President/CEO), disclose annually, in writing,
interests that could give rise to conflicts.
2.On November 1, 1999, the Organization adopted a Corporate
Compliance Program applicable to Directors, officers, key
employees, and all organization employees, which is inclusive
of a Conflict of Interest Policy. The purpose of
the Policy provides that the Organization is committed to
conducting its business ethically and in conformance with
all applicable laws, regulations, and standards.
Pt VI-B, Line 15
United Church Homes participates in the annual AAHSA-CEMO Leadership
Compensation Survey. The survey consists of multi-facility provider
organizations sponsored by the American Association of Homes and Services
for the Aging know as AAHSA. The database of this survey includes
125 organizations representing 1,946 executives.
As to other officers or key employees of the organization, the
process for determining their compensation is determined by the
CEO, not the Executive Committee, nor independent persons.
As such, the President & CEO normally relies on this same Compensation
survey report to determine to compensation package of the Vice
President/key employee positions.
