Schedule H, Part V, Facility Info, pages 4-6
The IRS instructions for Part V, state that the questions regarding community health needs assessments (lines 1-7) are optional for tax year 2010. Additionally, Section B, lines 8-21, are also optional for hospital orgainzations reporting a tax year beginning on or before March 23, 2010. Summit Healthcare Association (SHA) qualified for this optional reporting, and did not choose to respond to lines 1-21. However, the tax software program that was utilized in preparing the Form 990 automatically generated responses to questions: 1;8-14; 16; 18; 20 and 21. These responses were the result of a software malfunction and were unable to be removed before the filing deadline. Please disregard those responses, and note that SHA did not complete them due to the fact they were OPTIONAL. All lines 1-21, in part V Section B, should be blank.

Neal Thompson served as both the Board Chairman and the Interim Hospital Administrator for a portion of the year. His service as Administrator was temporary, and deemed critical during an interim time that the Hospital was left with a vacancy in this position. He was compensated $73,797 as Administrator, and $9,561 as the Board Chairman. As of the end of the year, he was no longer serving as Administrator. Because he was compensated as an employee, he was not considered an independent voting member of the Board, and was not included in the total on Form 990, Part 1, line 4.

Dr. Thomas Paxman is a Governing Board Member as well as a physician investor in White Mountain Imaging, the same company that Summit Healthcare Holds 51% interest in.Dr. Terrence Jones Cavanaugh served as a Governing Board Member until May, and also is a 2.5% physician investor in White Mountain Imaging, the same company that Summit Healthcare holds 51% interest in. He also provides contracted services to Summit Healthcare. John Corder is a Governing Board Member who owns an outside company that provides limited security services to Summit Healthcare. Travis Merrell is a Governing Board Member and an officer at National Bank where Summit Healthcare holds checking accounts and CD's. Mr. Merril has loan signing authority, however, does not do so on Summit Healthcare related loans.Roma Lee Hiatt is a Governing Board Member, and her son is an employee of Clin-Path Associates. Clin-Path has a contract with Summit Healthcare to provide pathology services and is listed as one the Hospital's highest paid independent contractors.Doug Wright, DO served as a Governing Board Member from May to December and was also contracted as an ER Physician Group employee.
