The purpose of this corporation, consistent with the objectives for which our tax exemption was filed, includes the following: 1. To acquire medical practices and develop new start-up practices to improve access to children's health services for all levels of economic status. 2. To employ quality physicians and other clinicians. 3. To practice medicine in conjunction with and in support of the patient care mission of Children's Hospital. 4. To develop, in conjunction with Children's Hospital, an integrated and geographically diverse pediatric health care delivery system. 5. To provide cost-effective, comprehensive pediatric care to the communities served by Children's Hospital and its affiliates through strong utilization review practices. 6. To ensure quality pediatric care delivery through strong quality assurance practices. Children's Hospital Medical Practice Corporation was incorporated on February 8, 1996, as a tax exempt, non-profit organization under the Louisiana Revised Statutes of Title 12 and the Federal Section 501 (c)(3) of the Internal Revenue Code. Serving the Community CHMPC participates in the State of Louisiana's KIDMED Program for indigent children. KIDMED is a Medicaid preventive program emphasizing Early and Periodic Screening, Diagnosis and Treatment (EPSDT) protocol. The screening component of EPSDT is represented by Louisiana KIDMED and includes medical, vision, hearing, and learning screening services. The CommunityCARE Program is in effect in all parishes of Louisiana. CommunityCARE is a primary case management program delivery system for Louisiana Medicaid and Louisiana Children's Health Insurance Program. All of our physicians and locations are enrolled in the CommunityCARE program with the exception of one location. Physicians employed by CHMPC demonstrate their commitment to the purpose of the corporation through numerous community activities. CHMPC employees are active in many educational forums including presentations at schools and community groups, serving as local board members, and disseminating information on various children's issues especially in the African-American communities. Kids First Clinics Kids First practices are fully staffed physician practices that are established in medically under-served neighborhoods throughout the city to serve the pediatric health care needs of the surrounding areas. Kids First Prytania, a start-up practice, was the initial endeavor of CHMPC. Kids First opened its doors on October 23, 1996. Dr. Keith Perrin, along with Dr. Carl Robinson, located this practice at 3600 Prytania Street to serve the population of Orleans Parish. This location is centrally located and served by public transportation. Dr. Sandra Robinson joined Dr. Carl Robinson following Hurricane Katrina. They added Dr Rachel Dawkins in 2008. They saw 7,388 patient visits in 2010, 96% of which were Medicaid. Kids First Ursulines and Kids First St Bernard were closed due to the physical damages to the clinics and the loss of community due to Hurricane Katrina. CHMPC has made a commitment to re-establish clinics to these communities should they rebuild and the population return. Kids First Louisa was re-opened in September 2009 after being closed since Hurricane Katrina. They were staffed with a half-time LSU physician, Dr Alice Farley. During 2010, they saw 809 patients, 97% of whom had Medicaid. A fifth Kids First practice was opened in March 2004 at 1661 Canal Street, in downtown New Orleans. Kids First TigerCARE is a collaborative effort between CHMPC and LSU Healthcare Network. The clinic is staffed by LSU pediatricians and serves as a training facility for LSU Residents. Kids First TigerCARE experienced 5,469 visits in 2010, of which 97% were Medicaid. A sixth Kids First practice was opened in October 2006 at 3321 Florida Avenue in Kenner, LA. Kids First TigerCARE Kenner is another collaborative effort between CHMPC and LSU Healthcare Network. The clinic is staffed by LSU pediatrician Dr. Edith Linares and also serves as a training facility for LSU residents. Dr. Linares and both staff members are fluent in Spanish and a majority of their patients are from the new Hispanic population that has come to the New Orleans area to work since the storm. Kids First TigerCARE Kenner experienced 2,028 visits in 2010 of which 81% were Medicaid. In 2008, we opened two new Kids First clinics, Kids First New Orleans East and Kids First Mid City. Kids First New Orleans East at 14401 Chef Menteur Hwy is in the predominately Vietnamese neighborhood, Village de l'Est in New Orleans East. Dr Tami Hinz saw 2,805 patients in 2010. 88% of whom had Medicaid. Dr Cecily Poree-Turner at Kids First Mid City at 4052 Ulloa Street in New Orleans had an existing practice and joined CHMPC in 2008. She saw 3,979 patients in 2010, 91% of whom had Medicaid. Other Clinics CHMPC not only serves the needs of under served areas of the City of New Orleans, but has also established other practices throughout the city to meet the pediatric health care needs of everyone. Effective April 1, 1997, Napoleon Pediatrics joined CHMPC. The clinic has six physicians practicing in two locations both accessible by public transportation. The first location, 2820 Napoleon Avenue is in New Orleans and the other at 3040 33rd Street in Metairie. The six physicians are Drs. Kirk Bellard, Sam Solis, David Estes, Keith Perrin, Tammy Senentz and Michael Heller. Dr Brandace Hider joined part-time in 2009. Patient visits in 2010 totaled 32,834 with 55% Medicaid for both locations. Effective October 25, 1999, Northlake Pediatrics joined CHMPC. The clinic is located on the north shore of Lake Pontchartrain at 725 W. 11th Street in Covington, LA. Drs. Margaret Strong, Ann Kay Logarbo, Pam Richard and Robert Faucheaux see patients at this clinic. Dr Stephen Mikell replaced Dr Richard in January 2009. Patient visits in 2010 totaled 14,240 with 16% Medicaid. Effective May 1, 2002, Lakeside Children's Clinic joined CHMPC. The clinic has six physicians practicing at 4740 S. I-10 Service Road in Metairie. This practice is located next to Tulane-Lakeside Hospital and is accessible by public transportation. The physicians are Drs. Jerry Casey, William Blake, III, Scott Zander, Elizabeth Adams, Keith Kappel and Claire Neumann. Dr Scott Bracey replaced Dr Casey upon Dr Casey's retirement. Patient visits were 22,609 with 45% Medicaid for the year ending December 31, 2010. On May 1, 2002, Physicians of River Ridge joined CHMPC. The clinic is located at 9605 Jefferson Highway in River Ridge and is accessible by public transportation. The physicians seeing patients at this location are Drs. Richard Doskey, John Firestone and Janine Lissard. Dr Doskey left the practice in May 2009. Patient visits were 8,475 with 69% Medicaid for the year ending December 31, 2010. On June 1, 2003, Klein Lawrence Pediatrics joined Children's Hospital Medical Practice Corporation. During 2009, two physicians, Drs. Reita Lawrence and Jaime Michel staffed the clinic. One office is located at 3100 Kingman Street, Suite 110 in Metairie and is accessible by public transportation. The second location is at 501 Rue de Sante, Suite 13 in La Place. 12,360 patients were seen, 55% Medicaid, for the year ending December 31, 2010 for both locations. Effective July 1, 2004, Ormond Pediatric Group joined CHMPC. Two pediatricians, Drs. Thomas Babin and Dolleen Licciardi, staff Ormond Pediatric Group. The clinic is located at 141 Ormond Center Court in Destrehan. In 2009, Ormond Pediatric Group experienced 6,486 patient visits. As of this time, they do not participate in Louisiana Medicaid. Effective January 1, 2006, Metairie Pediatrics joined CHMPC. Drs. John Barbara and Mark Morici and Cary Culbertson staff the clinic. Metairie Pediatrics is located at 2201 Veterans Memorial Boulevard, Suite 300 in Metairie and is accessible by public transportation. Patient visits were 22,070 in 2010 with 54% being Medicaid.

SchR_P05_S00_L01m Schedule R, Part V, Line 1m Children's Hospital Medical Practice Corporation is a wholly owned subsidiary of Children's Hospital EIN XXX-XX-XXXX. The purpose of this corporation is to complement and support the tax-exempt purposes of Children's Hospital with a separate corporation formed solely to maximize reimbursement from various providers, by allowing the separation of CHMPC physician billings under a separate tax ID number. The two corporations have the same President of the board. Audited financial statements are consolidated and all fund activites between both organizations are eliminated. Children's Hospital processes the payables and the payroll for CHMPC using its own funds. CHMPC collects patient receivables in its checking account and money is periodically transferred to Children's Hospital to offset the payments made by Children's Hospital on the account of CHMPC. Children's Hospital provides administrative services to CHMPC including legal, human resources, IT, accounting and tax return preparation without a cash reimbursement from CHMPC.
