ASC 740   Effective January 1, 2007, the Association adopted guidance included in ASC 740 Accounting for Uncertainty of Income Taxes, which prescribes a more-likely than-not recognition threshold and measurement attribute for the financial statement recognition and measurement of a tax position taken or expected to be taken. Under ASC 740, tax positions will be evaluated for recognition, derecognition, and measurement using consistent criteria and will provide more information about the uncertainty in income tax assets and liabilities. Based on an analysis prepared by the Association, it was determined that the application had no material effect on the recorded tax assets and liabilities or statements of operations of the Association.

Needs Assessment   Parma Community General Hospital has offered community health and wellness programs since 1986. Our mission is to provide the highest quality educational opportunities, information, resources and support to members of the communities. It is believed that when an individual maintains or improves their own health, they are also taking care of the community. The hospital's provides ongoing programs intended to increase access for members of our community to preventive, education and screening programs. Through surveys, referrals, assessments and by simply asking the question: "What do you want to know more about in regards to your health?" the Hospital develops programs targeting specific community needs. THE HOSPITAL WORKS WITH THE CUYAHOGA COUNTY BOARD OF HEALTH, THE ACADEMY OF MEDICINE, THE PARMA AREA COLLABORATIVE FAMILY SERVICES, THE LOCAL CITY SCHOOL DISTRICTS, THE Y.M.C.A., AND OTHERS TO COORDINATE COMMUNITY WELLNESS AND EDUCATION PROGRAMS. KNOWING WHAT EACH OTHER IS DOING AND HOW THEY APPROACH THE ISSUES THAT THEY CONFRONT HAS BEEN THE CORNERSTONE OF PARMA HOSPITAL'S EFFORTS TO ADDRESS SPECIFIC NEEDS.
Patient Education of Eligibility for Assistance   1. Uninsured patients arriving at our emergency department or other registration area are screened at the time of service by a financial counselor. Patients identified without insurance are given an application for Financial Aid. The patient will be given a brief explanation of the form and the Ohio's Hospital Care Assurance Policy (HCAP). The patient will be reminded to return the application with proof of income to the address printed on the form within two weeks. 2. Under the HCAP, Parma Community General Hospital offers basic, medically necessary hospital-level services free of charge to individuals who are residents of Ohio, and whose income is at or below the Federal Poverty Income Guidelines or who are currently eligible recipients of the Disability Assistance (DA) Program. If a self pay patient does not qualify for the HCAP program they may qualify for a sliding scale discount under Parma Hospital's financial assistance program. 3. To be eligible for HCAP you must be a resident of the State of Ohio and your family income during the 3 months or the 12 months prior to the date of service was at or below 100% of the Federal Poverty Income Guidelines. 4. Applications are mailed to all self-pay patients with a copy of the bill and to patients requesting financial assistance. Applications are available for patients in the Financial Counselor Office, Cashier's Office, Registration areas or Patient Accounting Office Monday through Friday, 9:00 a.m. to 4:00 p.m. 5. The Hospital has posted notice in the appropriate areas within the facility (Cashier's/Admitting area and Emergency Department) for the purpose of informing the public the criteria for eligibility and applications are available upon request. 6. The hospital also provides assistance to the patient to apply for Medicaid. 7. If a patient's family income falls between 101% - 400% of the Federal Poverty Income Guidelines they may qualify for a sliding scale discount. 8. The hospital accepts financial aid applications for services for 3 years from the date of receiving the proper notice that uncompensated services are available. 9. If the patient does not qualify for financial aid or Medicaid, it is the policy of the Hospital to offer uninsured patients a discount from charges to assist the patient in satisfying their obligation to pay for requested medical services and to reduce administrative and billing/collection cost. This policy is also designed to ensure that patients are charged a fair market price for services provided.
Community Information   Parma Community General Hospital is an independent, not-for-profit, community hospital founded in 1961 by six communities in southwestern Cuyahoga County - Parma, Parma Heights, Brooklyn, Brooklyn Heights, North Royalton and Seven Hills. The Hospital serves more than 350,000 people in southern Cuyahoga and northern Medina counties, and the predominant payor is Medicare at 60 percent. The 332-bed hospital has a medical staff of over 500 physicians in more than 30 specialties and a comprehensive campus of services. Accredited by the Joint Commission, Parma Hospital offers acute and sub-acute inpatient care including specialty centers for heart, cancer, orthopedics, pain management, acute rehabilitation and bariatric care. In 2010, Parma Hospital had 48,651 emergency visits and 15,365 admissions, 12,889 surgical/interventional procedures were performed. In order to make outpatient services more convenient and accessible, Parma Hospital offers Diagnostic Imaging at Ridge Park Square in Brooklyn, and WellPointe Pavilion, an outpatient facility that is home to therapy services, lab and imaging services including digital mammography, to expand care to Broadview Heights. Parma Hospital also maintains lab and therapy satellite facilities in North Royalton and Parma, adjacent to the hospital. Parma Hospital also staffs express clinics inside Discount Drug Mart stores in Independence and Olmsted Falls for quick, convenient treatment of basic illnesses. Community Building Activities Parma Hospital maintains relationships with many community groups, including Senior Centers and the Cuyahoga County Public Libraries. We have worked with the Parmatown Mall Walkers since 1999 offering blood pressure screenings and monitoring. Free screenings are offered throughout the year. The Hospital also offers free melanoma/skin cancer, prostate cancer, and colorectal cancer screenings. By working with the Parma City School District and the Y.M.C.A. of Greater Cleveland, a 12 week program entitled Club H.E.A.L.T.H. (Helping Each Adolescent Learn To develop Healthy Habits) was offered to the 5th and 6th graders of one of the Elementary Schools. The focus of this program was to address childhood obesity. The Hospital also provides support to Cuyahoga Community College's ENCORE Program and various "You and Your Health" classes. The Hospital works directly with Parma Health Ministry, a program supported by the Hospital's Foundation, to provide members of our community who are experiencing economic hardship with access to appropriate medical care. Care provided through the Ministry is provided free of charge and reported in the Hospital's charity amounts.
