    THIS PRIVATE FOUNDATION WAS ESTABLISHED IN LATE 2009,BUT NOT DID NOT RECEIVE ANY CONTRIBUTIONS UNTIL LATE 2010, AT WHICH TIME LIFE INSURANCE POLICIES WERE DONATED BY TWO INDIVIDUALS TO THE FOUNDATION. NO ADDITIONAL CONTRIBUTIONS WERE MADE IN 2011. THE AMOUNT OF ACTIVITY SUSTAINED BY THE FOUNDATION FOR THESE TAX YEARS WAS MINIMAL, WITH NO INCOME EARNED BY THE FOUNDATION AND NO DISTRIBUTIONS MADE. THE ONLY ACTIVITY DURING THIS TIME PERIOD CONSISTED OF THE TRANSFERRING OF ALL RIGHTS ASSOCIATED WITH THE POLICIES TO THE FOUNDATION. THE CPA WHO ASSISTS THE TAXPAYER IN TAX MATTERS WAS NOT AWARE OF THE EXISTENCE OF THIS FOUNDATION. ONCE THE EXISTENCE WAS DISCOVERED BY THE CPA IN EARLY 2012, THE CPA AND THE TAXPAYER HAVE WORKED DILIGENTLY TO GATHER THE INFORMATION NEEDED TO FILE THE NECESSARY RETURNS AS QUICKLY AS POSSIBLE.
