    In 1993, Terry and Rosann B. Douglass formed the Terry D. and Rosann B. Douglass Foundation herein, Trust as a tax-exempt trust FEIN 58-2068304. The name of the Trust was later changed to Provision Trust. The Trustees of the Trust determined that it was in the best interest of the Trust to convert its operations to the corporate form.
