All services are geared toward direct patient care. The Medical Center had 16,293 admissions and 72,023 patient days plus 225,287 outpatient visits and 4,385 same day surgery procedures. The Medical Center also provides other community services. The Student Health Awareness Center provides health education and information, as well helping girl and boy scouts to earn badges. The Clinic also provides specialty services such as OBGYN, pediatric, neurology, cardiology, and many others. CentraState established a community advisory council designed to access community health needs. The ER's "Buckle Up" program works with the local Police to check vehicles for seat belt use. An award winning program First Impressions helps new parents to cope with the change in their lifestyle.The Medical Center provides Charity Care to patients who meet certain criteria defined by the New Jersey Department of Health and Senior Services ("DOHSS") without charge or at amounts less than established rates. The amount of Charity Care provided during the year, based on charges the Medical Center established, was approximately $40.8 million.

  Part VI - Affilated Health Care System Roles and Promotion CentraState Healthcare System, Inc. is the sole member of the following not-for-profit corporations: CentraState Medical Center, Inc (the Medical Center), Center for Aging, Inc. d/b/a Applewood Estates, CentraState Healthcare Affiliates, Inc. d/b/a The Manor, CentraState Assisted Living, Inc. d/b/a Monmouth Crossing, and CentraState Healthcare Foundation, Inc. CentraState Healthcare System is the sole stockholder of CentraState Healthcare Services, Inc., a for-profit corporation. CentraState Healthcare Services owns all of the membership interests of CentraState Medical Arts building LLC, a limited liability company. CentraState Medical Associates, P.C. ("Med Associates") is a controlled subsidiary of the Medical Center.The Medical Center is an acute care hospital whose mission is to provide the highest quality patient care for the central New Jersey community it serves. The Medical Center has 281 licensed beds. Applewood Estates is a continuing care retirement community accredited by the Continuing Care Accreditation Committee. Applewood Estates consists of 290 independent apartments, 20 cottages, 40 residential health care units, and a 60 bed skilled nursing facility. One of Applewood's outreach programs entitled Seniors First (for adults 60 and over) financially subsidized by the Center for Aging, sponsors educational programs to the community that addresses health, lifestyle and safety issues, as well as health fairs and screenings. In addition, survivors and caregivers support groups for stroke and Parkinson's disease are among the resources available. The Manor provides skilled nursing services for the 123 elderly residential units including sub-acute, rehabilitation and I.V. therapy. Monmouth Crossing owns and operates an assisted living facility for the elderly consisting of 76 units. Residents with special needs and memory related diseases receive comprehensive, attentive services in a safe, secure environment where specialized activities and programming are provided. Low and moderate income housing needs are also accommodated at the facility. CentraState Healthcare Foundation was established for the purpose of soliciting and investing funds for the benefit of the Medical Center and other not-for-profit entities of the System. Recent examples of funds raised were for cancer care, multiple sclerosis, and diabetes programs contained within the Medical Center.
  Part VI - Explanation Of How Organization Furthers Its Exempt Purpose CentraState contributes to the community through substantial employee involvement with other charitable and public organizations such as Girl Scouts of the Jersey Shore, Work force Investment Bureau of Monmouth County, Veterans Community Association, Fingerprint America, Sunrise Optimist Club, Monmouth-Ocean Development Council, Muscular Dystrophy Association, Freehold Township, Monmouth County child Advocacy Center, the National Organization of Women Business Owners, Rutgers University, Cancer Institute of New Jersey and the College Advisory Boards of Monmouth County Community College and Brookdale Community College. Efforts include increasing services to cancer survivors, coordinating efforts for fund raising and organizing student training programs. CentraState also provides additional community benefit through its 670 volunteers who contributed over 58,000 hours of service to their community through opportunities in every hospital department. Volunteers significantly enhance patient and family support in the following service categories: Blood Cart, Dietary Menu, Emergency Department Lounge, Flowers, Gift Shop/Cart, Hospitality Cart, Information Desks, Mail or Messenger, PRN/Floaters, Pastoral Care, Animal Assisted Therapy, Snack Bar, Surgery Waiting Area, Patient Transport, Hair Care, Service Learning.
  Part VI - Community Building Activities Community building activities include Childhood Development Center which provides public access to daycare needs $235,970, Cancer Navigation System which provides support and information to members of the community with cancer $52,402, Community Coalitions $6,871, Economic and Workforce Development $15,238, Camp Quality support for kids with cancer $11,519, Girl Scouts support $1,701, American Heart Shoreline Walk and Walk to Remember $1,263.
  Part VI - Community Information The community in which CentraState serves is concentrated in Monmouth and Ocean counties. The average household income in the service area is $120,215. The median household income is $88,158 and the median age is 39. The total population in the service area for 2011 is 354,334, with 121,762 households. The racial mix of the service area population includes 78.8% white, 6.6% Asian, 4.0% black, 9.2% Hispanic, 1.3% multi-racial and .2%other. Gender mix is represented by 50.8% female and 49.2% male.
  Part VI - Patient Education of Eligibility for Assistance Under the Financial Assistance and Financial Discount Policy for the Uninsured, CentraState notifies patients of the existence of Financial Assistance through the posting of signs in English and Spanish in all intake areas and Patients are also notified of the existence of financial counseling and Financial Assistance through notification on bills. Patients and their families are offered financial counseling before, during or after their services. Scheduled elective inpatients and outpatients are contacted as soon as possible prior to scheduled services. Inpatients are provided financial counseling as soon as possible during inpatient stays, on regular business days. For any scheduled elective inpatients or outpatients or any inpatients not counseled during their stay, and for all other patients (including Emergency Department patients) financial counseling are offered by phone or in person. Patients are provided, on each bill, a phone number to call a financial counselor.
  Part VI - Needs Assessment CentraState Healthcare System is a charter member of the Health Improvement Coalition of Monmouth County (HICMC), formerly known as the Monmouth County MAPP Coalition. This collaboration completed an initial Community Health Improvement Plan (CHIP) for Monmouth County in April, 2007. A new/revised CHIP was completed in 2011. CentraState participated in the process as well as hosted regular meetings. Specific health needs of the population were identified and prioritized. During 2011, strategies to address the prioritized needs were developed and will be implemented in 2012-2016 by the HICMC members including CentraState in its service area. The CHIP is to be released in printed form in Spring 2012 at a county-wide public meeting and made available on the Monmouth County website by Summer 2012.Additionally, the Freehold Community Alliance for the Prevention of Substance Abuse (Freehold Twp and Freehold Boro) conducts a needs assessment every three years in which CentraState participates. That assessment is scheduled for 2012. CentraState utilizes the results at the CentraState Health Awareness Center (HAC) and partners with the Alliance to do programming, particularly in the local schools.CentraState is developing a plan to complete a CentraState Community Health Needs Assessment as required by the Affordable Care Act and in accordance with section 501(r) of the Internal Revenue Service Code. A steering committee was formed in 2011 and CentraState will conduct a complete Community Health Needs Assessment and adopt an implementation strategy to meet the needs identified by the assessment in 2013. The plan will be made widely available including the CentraState website as well as distribution of printed copies.
Number of Hospital Faciltiy - 0 Part V, Line 19d - Other Billing Determination of Individuals Without Insurance The hospital charges uninsured patients 115% of the Medicare rate if the patient is under 500% of poverty level or if the patient's income is unknown. If the patient's income is greater than 500% of poverty level, the patient is charged 150% of the Medicare rate. In addition, if the patient meets income and asset criteria, further full or partial charity reductions are applied to the discounted rate.
Number of Hospital Faciltiy - 0 Part V, Line 13g - Other Means Hospital Facility Publicized the Policy The existence of the Financial Assistance policy and how to contact the facility to apply, for insured and uninsured patients, is noted on at least 3 patients bills sent to the patient/guarantor prior to collection activity. Please note for answers 13a and 13 b-f, the postings provide patients with information regarding the existence of the Financial Assistance policy and various levels of income/asset criteria detail, but not the Financial Assistance policy itself.
  Part III, Line 9b - Provisions On Collection Practices For Qualified Patients Patients are billed routinely for unpaid balances and shall be notified of any impending collection or legal action. Patients are provided with contact information to discuss any questions or problems. Patient bills indicate the availability of payment schedules. Upon patient request, the following payment schedule for balances of $100 or more may be agreed upon: Balance of $100 - $450 - Monthly payment = 1/4 of the balance, $451 - $3,000 = 1/12 of the balance, over $3,000 = minimum of $250 per month or 1% of gross income/month, whichever is more. If the patient and Medical Center representative mutually agree to a payment schedule and payments are received as agreed, further collection action will not be taken. Exceptions to the above schedule may be made by Patient Financial Services Management upon the request of the patient, and documentation by the patient of inability to pay according to the established schedule.
  Part III, Line 8 - Explanation Of Shortfall As Community Benefit CentraState Medical Center feels that Medicare underpayments (shortfall) are community benefit, and associated costs. CentraState believes that these services and related costs promote health of the community as a whole and are rendered in conjunction with the organization's charitable tax-exempt purposes and mission in providing medically necessary health care services to all individuals in a non-discriminatory manner without regard to race, color, creed, sex, national origin, religion or ability to pay and consistent with the community benefit standard promulgated by the IRS.The American Hospital Association ("AHA") feels that Medicare underpayments (shortfall) are community benefit and thus includable on the Form 990, Schedule H, Part I. CentraState agrees with the AHA position for the following reasons: Providing care for the elderly and serving Medicare patients is an essential part of the community benefit standard. Medicare, like Medicaid, does not pay the full cost of care, and many Medicare beneficiaries, like their Medicaid counterparts, are poor. More than 46% of Medicare spending is for beneficiaries whose income is below 200% of the Federal poverty level. Medicare underpayment must be shouldered by the hospital in order to continue treating the community's elderly and poor. These underpayments represent a real cost of serving the community
