Health Care & ManagementFunds received were used to provide funding for the exempt purpose of general patient care. The Memorial Hospital is an acute care, not -for-profit teaching community health institution and research facility serving the Blackstone Valley community of Rhode Island and Massachusetts. Memorial, which is located in Pawtucket, Rhode Island, is a teaching affiliate of the Warren Alpert Medical School of Brown University and the chief site for the medical schools primary care academic program housed in the Center for Primary Care. Memorials goals are to provide quality multi-disiplinary services, to see patients regardless of race, color, national origin, socioeconomic status or place of residence and to act as an educational resource for patients, physicians, nurses and the community. Memorial provides care to patients that meet certain criteria under its charity care policy without charge or at amounts less than its established rates. Because Memorial does not pursue collection of amounts determined to qualify as charity care, they are not reported as revenue. Memorial provided 30,098patient days of service in the year ended September 30, 2011. Memorial had 6,373 admissions with an ALOS of 4.72 days. Memorial operates a Level 2 trauma center ER, which serves patients regardless of ability to pay. For the year ended September 30, 2011, Memorial provided its patients with 32,195 ER visits. Primarily through the Department of Family Medicine, Memorial attempts to integrate patient care within the context of the patients family, and, whenever possible and appropriate, care for the family as a unit. Memorial also seeks to cooperate with community organizations in the management of patient care. The Department of Family Medicine follows patients in whatever setting they present, whether home, nursing home or tertiary care center. To help accomplish this goal, Memorial has a Homecare Department which works with the Department of Medicine, Nursing, Social Services, Physical Therapy, Utilization Review, Medical/Surgical Supply and Pharmacy to ensure that patients leaving the inpatient services receive continuous and appropriate care after leaving Memorial. Memorial has well developed programs in Occupational Health, Physical Therapy and Medical Rehabiliation. Memorial's research is internationally recognized. Many of the materials and procedures developed are being used all over the country. Two of the many widely recognized programs at Memorial are the Osteoarthritis Initiative and the Women's Health Initiative. Memorial's research efforts are guided by the goal of working to address unmet health needs in the community. Memorial's Department of Family Medicine accepts approximately 13 residents per year for a total of 39 residents in the training program. This program is nationally competitive, matching with high qualit residents in spite of the decreasing interest of medical students in primary care specialties. The Department of Family Medicine has developed a model teaching community health center at two centers which serve a large number of medically indegent patients in the Blackstone Valley. These models of community based education, research and patient care serve as training sites for residents who desire a career in rural or urban community health centers.

The Emergency Department Clinicians are edudated in regard to proper refferal process. If the patient states they are uninsured and need to seek Charity Care through discussion with the clinician, then they will be reffered to the Family Care Center at Memorial Hospital. We pre-screen all self-pay Emergency Department patients for Community Free Services. If they pass the screening they are sent o the Financial Councelor for assistance in filling out any necesssary forms. SchH_P06_S00_L04 Schedule H, Part VI, Line 4 The Following Cities and towns in the state of RI are within the primary service area of the hospital: Pawtucket, Central Falls, Cumberland, East Providence, Riverside, Rumford and Lincoln. In addition, the hospital serves the following cities and towns in Massachusetts: Attleboro, Seekonk, North Attleboro and Plainville. The majority of the patients served during the current fiscal year were from the city of Pawtucket (59%) followed by Central Falls (16%). The city of Pawtucket has a population of approximately 71M and is the forth largest city in the state. Pawtucket was a major center of manufacture of cotton textiles during the American Industrial Revolution. Slater Mill, built in 1793 by Samuel Slater in downtown Pawtucket, was the first fully mechanized cotton spinning mill in America. The textile business in Pawtucket declined during the 1930s with many manufacturers closing or moving their facilities. As of September 2011 the city's unemployment rate was 12.1%. According to the 2010 census, the median income for a household in the city was $31,775, and the median income for a family was $39,038. Males had a median income of $31,129 versus $23,391 for females. The per capita income for the city was $17,008. About 14.9% of families and 16.8% of the population were below the poverty line, including 24.5% of those under age 18 and 15.2% of those age 65 or over. The racial makeup of the city is 75.39% White, 7.31% African American, 10.75% from other races, and 5.34% from two or more races. Hispanic of any race were 13.9% of the population. 20.6% of Pawtucket residents are French or French-Canadian descent, and 11.6% of Portuguese descent. The hospital operates the Notre Dame Ambulatory Center in Central Falls, RI. Central Falls has a population of 19,042, and represents approximately 16% of the hospital's discharges. The median income for a household in the city was $22,628, and the median income for a family was $26,844. About 25.9% of families and 29% of the poulation are below the poverty line, including 40.8% of those under age 18 and 29.3% of those age 65 or over. The unemployment rate was 14.6% in September 2011. The racial makeup of Central Falls is 51.16% White, 5.82% African American, 28.35% from other races, and 7.38% from two or more races. Hispanic or Latino of any race are 47.77% of the population.
SchH_P06_S00_L05 Schedule H, Part VI, Line 5 The mission of Memorial Hospital is primary care and preventative medicine, providing advanced diagnosis and treatment with a focus on teaching and research. In order to achieve it's mission, the hospital sponsors several activities throughout the year. in FY 2011, the hospital sponsored the foloowing activities: Summer Food Safety, Blood Drive, Look Good - Feel Better, AARP Safety Driving, Hearing Screening, Emerald Bay Manor, Cholesterol/Glucose Screening, Annual Health Fair, Healthy Kids Fair, Leon Mathieu Center Fair, Progeso Latino Fair, PAWSOX, Pawtucket Rotary, College Crusade, and several fund raising "walks". Emergency Preparedness Activities- Memorial Hspital works with state and local partners in the planning and execution of disaster readiness and public health emergency activities. On a state level, MHRI is a member of the Hospital Association of Rhode Island (HARI) and participates in monthly hospital preparedness meetings with other acute care hospitals, community health centers, and the RI Department of Health. Additionally, local emergency management representatives from the five cities and towns that make up the hospital's catchment area participate in hospital Emergency Management Committe Meetings, held monthly. Local partners in conjunction with police, fire, and emergency medical services participate in hospital preparedness trainings such as evacuation, infant abduction, and severe weather scenario drills, Local partners have also worked in partnership with the hospital to support events such as hazardous material release, severe weather events, and mass vaccinations.
SchH_P06_S00_L06 Schedule H, Part VI, Line 6 Other information- Memorial Hospital of Rhode Island is a 294-bed acute care, not-for-profit, teaching, community health institution providing health care to a population of approximately 300,000 residents in Pawtucket and the surrounding Blackstone Valley region of Rhode Island as well as portions of southeastern Massachusetts. The Hospital offers a comprehensive array of medical, surgical, pediatric, obstetric and ancillary services. In addition, the Hospital has developed specialized expertise in areas such as Oncology, Rehabilitative Medicine, Critical Care Medicine, Cardiology, Pulmonary Medicine, Endocrinology, Hematology, Geriatric Medicine, Infectious Disease and Primary Care. Altogether, there are 34 medical and surgical specialties represented by the physicians on the Active Staff of the Hospital. The mission of Memorial Hospital of Rhode Island is primary care and preventative medicine, providing advanced diagnosis and treatment with a focus on teaching and research. An essential resource in achieving this mission is the medical expertise provided by the Hospital's competent and caring professional staff. The Hospital strives to achieve the timely and efficient delivery of care in a compassionate atmosphere. The vision of the Hospital is to excel as a primary health care network and community teaching hospital requiring it to: * Continuously improve the quality, maximize the value, and expand access to patient services; * Build a learning organization by creating an environment and structure that fosters role development; * Support research efforts and translate research findings into standards that measure and improve patient care for high volume and/or underserved vulnerable populations within its community. Inpatient services are provided in 227 medical/surgical beds, an 18-bed intensive/coronary care unit, a 17-bed rehabilitative medicine unit, a 22-bed pediatric unit and a 13-bed obstetrics/gynecology unit. Comprehensive outpatient services are provided at the Pawtucket campus by the Center for Primary Care and Prevention, the Ambulatory Care Center ( outpatient specialty care and numerous clinics ), a 24-hour Emergency Department, a Hospital-based ambulatory surgery center, an endoscopy department, rehabilitation services, a cardiac catheterization and testing lab, radiology and phlebotomy services and a multitude of physician offices/clinics providing specialty care. Primary care, urgent care, ambulatory surgery, pain management, physical and occupational therapy, and occupational health services are also provided at the Notre Dame Ambulatory Center in Central Falls, Rhode Island. This center is located on the campus of the former Notre Dame Hospital. This facility became part of the Hospital in October 1989 and was converted to a comprehensive ambulatory care center. Memorial Hospital serves as the second largest teaching hospital in the state of Rhode Island. The Hospital is formally affiliated with Brown University School of Medicine. Through this affiliation which commenced in 1969, the Hospital has been able to strengthen its commitment to provide education and research programs within the community health institution milieu. The Hospital is one of eight charter hospitals included in the Brown University program. A top priority of the Hospital has always been the integration of medical students and medical residents into the health care delivery system. The interaction with medical students and residents helps keep all members of the Hospital's community abreast of the latest developments in medicine. The Hospital sponsors medical residency programs in the following areas: Family Medicine ( 39 residents ), Internal Medicine ( 30 residents ), Pathology ( 1 resident ) and Podiatry ( 2 residents ). Additionally, fellowships in Hematology ( 1 fellow ), Infectious Diseases ( 1 fellow ), Pulmonary Medicine ( 2 fellows ), Maternal and Child Health ( 2 fellows ) and Pediatric Neuropsychiatry ( 1 fellow ) are sponsored by the Hospital. Training programs in Nursing, Lab Technology, Nurse Anesthesiology, Physician Assistants, Pharmacy and Physical Therapy are also based at the Hospital. The Brown Medical School Residency in Family Medicine at the Hospital is considered to be one of the top programs in the country in this primary care discipline. The preventative medicine focus of the department is evidenced by various on-going research projects. Preventative medicine serves an integral role in the discipline of Family Medicine and, accordingly, it remains a key component of the Hospital's Family Medicine residency program. Memorial Hospital is also home to the Brown Medical School Primary Care Internal Medicine Residency Program. As a result of the partnership agreement between the Hospital and the Brown University School of Medicine, the Hospital is designated as the University's Center for Primary Care and Prevention. This center serves as the focal point for university-wide collaboration efforts in primary care, research, education and practice. The Hospital's Department of Medicine has many faculty who hold appointments at the Brown University School of Medicine and work on a full-time basis to educate medical students and house staff physicians. The Hospital's active medical staff also includes a large group of clinicians in private practice who volunteer their services to medical education. Together, these faculty create a complementary staff of expert physicians and scientists. Research Research is also a core value of Memorial Hospital. The research interests of the staff at the Hospital cover a wide area of medical disciplines. There is, however, one major thrust at the Hospital which is common to all of the research programs operating at the Hospital. This common thrust is to make the Hospital a leader in primary care and preventative medicine. There are over one hundred and thirty ( 130 ) active research studies at the hospital funded by nearly $6.5 million in annualized grant revenue. These studies are funded through a variety of federal, state and private grants.

F990_P06_S0B_L11b Form 990, Part VI, Section B, Line 11b Once the independent audit is complete, the Controller of the organization gathers all the necessary information from various employees, departments and governing members. Several schedules are prepared to tie out different line items of the Form 990. The Controller then utilizes an on-line software to complete the 990. The software used ensures that all required fields and supplemental schedules are attached. The software verifies when the Form is complete and there are no errors. The draft 990 is then printed and submitted to the President for review. Copies are made and presented to each board member for their review. All disscussion of the 990 is documented in the minutes. The President informs the Controller of any changes or additions to the Form that result from the board review. The Controller makes all necessary changes and resubmits to the President for signature. Once signed, the 990 is electronically submitted to the IRS.
F990_P06_S0B_L12c Form 990, Part VI, Section B, Line 12c Every Board member, volunteer and employee is required to complete a Conflict of Interest Form on an annual basis. Any conflicts of interest must be disclosed. Signatures are required on all forms.
F990_P06_S0B_L15 Form 990, Part VI, Section B, Line 15 The organization has an Executive Committee which also acts as a Compensation Committee for the compensation of the CEO. In order to determine compensation, surveys are conducted for area CEOs. The committee also utilizes 990s from other hospitals in the state of RI. The compensation is approved by the Board of Directors, and a written employment contract is signed. The CEOs salary is reviewed each year, and is usually increased by a percentage no higher than the employees of receive from the hospital.
F990_P06_S0C_L19 Form 990, Part VI, Section C, Line 19 The Annual Reports for the hospital are posted on the organization's website. It is noted on the website that the hospital has a Conflict of Interest Policy and that copies are available upon request. The phone numbers of the Public Relations Department are posted on the website as well. The PR Department has been instructed tho direct any requests for Audited Financial Statements and or 990s to the Controller's office. Both are made available in hardcopy or electronically.
F990_P11_S00_L05 Form 990, Part XI, Line 5 The Additional Minimum Pension Liability accounts for the majority of the decrease in Other Changes in Net Assets in the amount of $12,124,629. Unrealized losses account for the rest.
