SUPPORTED ORGANIZATIONS, SCHEDULE A, PART I, LINE 11H(VII), IT WAS NOTED ON THE 2009 FORM 990 THAT THE ORGANIZATION'S ARTICLES OF INCORPORATION WERE BEING REVISED TO INCLUDE ALL OF THE ORGANIZATIONS LISTED IN PART I, LINE 11(H) AND THE REVISIONS WERE TO BE SUBMITTED TO THE BOARD OF DIRECTORS FOR APPROVAL PRIOR TO JUNE 30, 2011. DURING REVIEW OF THE ORGANIZATION'S ARTICLES OF INCORPORATION, IT WAS DETERMINED THAT GREATER EFFICIENCIES COULD BE CREATED BY MERGING THE ORGANIZATION INTO SPECTRUM HEALTH UNITED. THE MERGER WAS APPROVED BY THE SPECTRUM HEALTH HOSPITALS GOVERNING BOARD AND WAS COMPLETED FEBRUARY 1ST, 2012.
