AT THE FEDERAL LEVEL, DURING FY 2011, THE CENTER ADVOCATED FOR INCREASED MEDICARE REIMBURSEMENT UNDER THE CANCER CENTER RULES AND ADVOCATED FOR INCREASED RESEARCH FUNDING FOR THE NIH AND NCI. QUESTION 2 - NEEDS ASSESSMENT As a dedicated Comprehensive Cancer Center, the only health need we address is cancer-from education to prevention to diagnosis, treatment and survivorship. For the purpose of Fox Chase Cancer Center as a whole, our community is anyone who seeks the services of Fox Chase, whether they live near us, in another state, or even in another country. Department of Health and tumor registry data to identify zip codes in surrounding Philadelphia, Montgomery, Bucks, Delaware and Chester counties that appear to have the most need for community education and screening. Also, as currently required by the legislation, we are gearing up for the first mandated comprehensive needs assessment which for us must be completed by June 30, 2013. QUESTION 3 - PATIENT EDUCATION FOR ELIGIBILITY FOR ASSISTANCE From FCCC Charity Care Policy: Mission TO PREVAIL OVER CANCER MARSHALING HEART AND MIND IN BOLD SCIENTIFIC DISCOVERY, PIONEERING PREVENTION, AND COMPASSIONATE CARE. Procedure When a patient calls in to schedule an appointment, the New Patient Office will notify Patient Financial Services (PFS) of anyone who is either a.) Not insured b.) Participates in a health plan that FCCC does not accept or is out of network or c.) Communicates a concern regarding the ability to meet FCCC obligations (i.e. under-insured, limited coverage, etc.) All such patients will be contacted by a Financial Counselor. No financial assistance will be extended to a patient until a plan of treatment has been established. QUESTION 4 - COMMUNITY INFORMATION 75% of Fox Chase Cancer Center's patients come from a nine county region across Pennsylvania and New Jersey. The five counties in Pennsylvania are Bucks, Montgomery, Chester, Delaware and Philadelphia. The 4 counties in New Jersey are Mercer, Burlington, Camden and Gloucester. QUESTION 5 - COMMUNITY BUILDING ACTIVITIES Leadership Development and Training for Community Members The Office of Health Communications and Health Disparities (OHCHD) at Fox Chase Cancer Center makes substantial contributions to the community in a variety of ways each year. One mechanism OHCHD uses to make positive contributions to the communities in the region is by building the capacity of other organizations to reduce cancer health disparities among the underserved through training, support and technical assistance. In prior years we addressed this need through the Partnership Program of the Atlantic Region Cancer Information Service (CIS) that was hosted within OHCHD. Since that program was terminated in January 2010, we have continued some of those efforts on our own. During FY 11, we: - Trained over 200 people from organizations planning to submit grant requests to the Komen Foundation and others on topics including using What Works, Adapting Evidence Based Programs, Grant Writing, and Designing and Implementing Evaluations. - Developed a new statewide Patient Navigation network and provided training on topics identified by participants - Participated in Comprehensive Cancer Control efforts in Pennsylvania by Continuing to work closely with community organizations/coalitions addressing cancer in underserved communities such as the Northern Appalachia Cancer Network; ATECAR--Asian Tobacco Education Cancer Awareness Research Program; and the Greater Philadelphia Breast Cancer Coalition. The OHCHD also undertook other leadership development and training activities, the costs of which are captured in the "Research" line item. For example, in the Body & Soul program, a faith based program to increase consumption of healthy foods, we both trained church leaders directly how to implement this program and trained community partners how to teach church leaders. We also provided training to churches on an ongoing basis to help them develop the ability to implement successful measures. In FY 11, we disseminated the program to 48 churches and affected over 1400 people with healthy eating messages and activities. Coalition Building Through its Office of Health Communications and Health Disparities, Fox Chase Cancer Center has developed a community outreach program designed to provide cancer education and screening to target minority populations experiencing health disparities, with a focus on African American, Latino/Hispanic and Asian communities. Part of this effort additionally includes educating these communities about clinical trials and other biomedical research opportunities. Called the Neighbors Initiative, this effort focuses on 11 zip codes near Fox Chase that house a high percentage of minority racial and ethnic populations. While we do work in other areas as well, the Neighbors area receives more concentrated attention. Entre into local diverse communities is contingent upon our building strong partnerships and relationships with community organizations and leaders who are respected by their fellow community members. In FY 11, we signed a Memorandum of Understanding with ENON Tabernacle Church, a 22,000 member, primarily African American congregation, to provide a variety of programs and events. We also sat on the planning committee for an annual Praise is the Cure event for African American cancer survivors and women and an event for Latina/Hispanic women. We also worked to link community organizations and leaders to other community organizations and leaders who share similar visions to increase their capacity to achieve desired change. In FY 11, we developed and disseminated a monthly e-newsletter to members of the PA Comprehensive Cancer Coalition that highlighted both relevant cancer news and upcoming events/situations/legislation needing attention from this particular community. The PA Navigator Network is also an effort to build a strong coalition of patient navigators across the state who can advocate for policies and services that benefit patients. Community Health Improvement Advocacy Community-based Cancer Screening: One aspect of our community health improvement effort is providing cancer screening in the community. Screening provided includes breast, skin and prostate. Most, but not all, community screening is accomplished on a mobile mammography van that goes where it is needed. A limited amount of screening takes place at partner sites as feasible (skin and prostate only). The entire community screening operation is based out of Fox Chase Cancer Center, including both the outreach/scheduling aspect and the clinical aspect. For mammography screening, which is by far the bulk of the community screening effort, the outreach and scheduling function is housed in the Office of Health Communications and Health Disparities, and the clinical function (which also includes costs related to the van) is housed in the Mammography Department of the hospital. The mobile mammography program services both corporate and community clients. In FY 2011, 3,689 mammograms were performed on the mobile van; 2,430 for women at corporate sites
