Management Employees and members of the Board of Directors may be asked to submit an additional form pertaining to a specific issue if, in the President's judgment, a vote is to be taken on a key matter on which conflicts may exist.The form shall be completed and kept on file. Failure of any management employee or any member of the Board of Directors to report conflicts of interest or failure to complete the form when required is a violation of the Code of Ethics of District 1199 WI and may subject an individual to discipline or termination.
