Homeless Management Information System (HMIS) - - - HMIS is a computerized data collection tool specifically designed to capture client-level, system-wide information over time on the characteristics and services needs of men, women, and children experiencing homelessness. HMIS allows the aggregation of client-level data across homeless service agencies to generate unduplicated counts and service patterns of clients served. Coalition for the Homeless of Houston/Harris County is the local HMIS lead agency. The HMIS software solution used in Houston/Harris County CoC is ClientTrack by Data Systems International. During FY 2011 (June 2010-July 2011), there were 80 participating agencies in HMIS with 157 programs and 617 users. The HMIS data was used for the 2010 Annual Homeless Assessment Report (AHAR), which was submitted to HUD in January 2011. Emergency Family category was accepted for the first time in AHAR, average bed utilization across accepted categories was 82.4%, and missing data rates on average less than 20%. During the second quarter 2011, Houston/Harris County started participating in the quarterly HUD PULSE report - the only CoC in Texas to do so. At the same time, two VA-funded program types serving homeless veterans (Grant-Per-Diem and Community Contract) joined the HMIS. All eligible agencies assisting clients through the Homelessness Prevention and Rapid Re-Housing Program (HPRP), funded through the 2009 American Recovery and Reinvestment Act, entered data in HMIS and sent quarterly reports to HUD. The Coalition HMIS support staff provided two new user sessions per month, as well as numerous refresher trainings, site visits, and webinars, which focused on data quality and privacy/security standards.

System Management Homeless - Continuum of Care Lead Agency Activities July 2010 - June 2011 - Community Planning for Prevention and Ending of Homelessness - Convened community stakeholders to determine priorities to implement in preparation for the HEARTH Act implementation and began planning process. - With public recipients of HUD funds, developed a structure by which to coordinate public funding and policy. - Conducted interviews with more than 30 homeless service providers for input on the Capacity and Gaps in the Homeless Residential and Service System.

Continuum of Care - - - The Coalition for the Homeless is the Lead Agency for the Continuum of Care (CoC) Homeless Assistance Grant application released by the U.S. Department of Housing and Urban Development. The Continuum of Care is a set of three competitively-awarded programs created to address the problems of homelessness in a comprehensive manner with other federal agencies. Those programs include, Supportive Housing Program (SHP), Shelter Plus Care (S+C) and Single Room Occupancy (SRO). When HUD publishes a Notice of Funding Availability (NOFA) for Continuum of Care Homeless Assistance in the Federal Register, applicants must submit specific information about a proposed project, along with their Continuum of Care application. Leading up to the application submission is notifying current participants via email and prospective new applicants via the media. Once all interested parties are notified two mandatory conferences are held, one for renewal programs and one for new programs. The conferences go over any and all changes and requirements indicated by HUD and the Coalition as well the expectation and requirements for those interested in submitting for new grants. Once new grant applications are submitted a public hearing is held so that each applicant who wants to submit for a new grant is given an opportunity to describe their program to the selection committee. The selection committees consist of eight participants, two from the City of Houston, and 2 from Harris County and four from the community who do not presently have grants through the Continuum of Care. The committee reads and ranks each new submission. As a result of the ranking the highest scored are selected to be submitted to HUD for grant approval. The Coalition and grant writers complete a review of each application. Agencies are notified of the corrections and changes that are to be made and applications are reviewed again. The new grant applicants are notified of their program ranking, those with the highest ranking application will be submitted to HUD. Once all applications are reviewed, applications are entered into HUD's electronic application and grants management system (esnaps) and submitted to HUD. This system supports the collaborative application process (CoC) Homeless Assistance Competition. HUD later notifies approvals of grants. The CoC Submitted a total funding request of $21,845,139 from fifty-three (53) programs for the 2010 application year. -- $14,585,616 - Renewal SHP projects -- $3,988,764 - Renewal S+C projects -- $3,270,759 - New SHP projects The CoC Submitted three (3) new programs for permanent housing applications and one (1) HMIS program were submitted through the Supportive Housing Program of which one (1) program targets chronically homeless and one targets homeless veterans. This increased the Houston/Harris County CoC permanent housing to: * 61 new units of permanent supportive housing * 85 new beds * 61 additional adults served * 24 additional children served

Other Program Services and Accomplishments - 2011 Enumeration and Community Needs Assessment The Coalition established a Point in Time Count. The methodology is based on the approved observational count model. The methodology included partnership with the City of Houston Health and Human Services department and the Coalition for the Homeless. Volunteerism was increased over 200 percent including 350 community volunteers to canvass 9 study areas in the CoC region. The model was accepted for presentation as a best practice at the Texas Conference on Ending Homelessness in October 2011 and determined a 25% increase in the homeless population from the 2010 Homeless Count. - - - Community Outreach - - - The Coalition's Supportive Housing Task Force engaged the services of the Corporation for Supportive Housing (CSH) to conduct extensive research on Permanent Supportive Housing needs and goals for the Houston/Harris County CoC. Extensive research was conducted with the city, county, housing developers, stakeholders, and service providers to produce the Permanent Supportive Housing Program and Financial Model on February 3, 2011. A Community Workshop on Supportive Housing was conducted on Tuesday, October 26, 2010 which was attended by seventy-five key stakeholders. The research demonstrated a need of 9,700 permanent housing units through the duration of the strategic plan and set an immediate goal of 1,500 additional PSH units in operation by 2016. In September 2010, the Coalition initiated a collaboration with the Houston Police Department to launch the Homeless Outreach Team (HOT) with support from the Downtown District. The homeless outreach team would serve in a process of coordinated intake. The HOT team was launched in January of 2011 and has placed 80 homeless persons into housing within the first 6 months of operation. November 14-20 of 2010, the Coalition Co-sponsored National Hunger and Homeless Awareness week with the My Fellow Man Alliance and the Houston Food Bank which resulted in seven advocacy events including a media event at the BEACON and the Oxfam Hunger Banquet demonstration at Rice University with partnership from City Council member Wanda Adams. On August 27, 2010 and November 30, 2010, The Coalition presented two free all day Disaster Management Conference for the Homeless Residential System with funding provided by Chevron. The conference provided immediate enrollments into the Transportation Assistance Registry program for all CoC housing providers, Continuity of Operations planning, and Readiness plan for homeless populations. The Coalition completed the Disaster plan for homeless populations and established an email and community voicemail alert system. The Annual Meeting was conducted on September 16, 2010 at the Junior league to maximum capacity sold out audience and included the Excellence awards for Housing/Re-housing, Specialized Outreach to Chronically Homeless, Prevention/Early Intervention and Achiever of the Year. The Annual report was distributed noting the developments and systematic changes of the HEARTH Act. - - - Homeless Court - - - Homeless Court (HC) is a special Court session for homeless defendants to resolve outstanding misdemeanor offenses and warrants within the City of Houston's Municipal Courts. HC builds on partnerships between the court, local shelters, and service agencies. It works to resolve the problems that homelessness presents with practical solutions. Initial referrals to HC originate in shelters and service agencies. Referrals are submitted to the Coalition for the Homeless of Houston/Harris County. Court staff pulls dockets, and court dates are set by the presiding judge. HC is designed for efficiency. From July 2010 through June 2011 793 clients were referred. 607 were successfully processed through the system and 139 were no shows for court. In all 2,448 cases were resolved. - - - Community Voice Mail - - - CVM served 3,909 people through 145 human service programs. The average number of clients added monthly was 171. 88% achieved one or more of their goals, including: employment, housing, healthcare, safe communications and access to social services. 64% of CVM clients reported "no financial resources", 11% of CVM clients reported receiving "public benefits". $659.32 was the average monthly income. - - - Information and Referral - - -3,463 unduplicated information and referral calls were received. - - - Consumer Advisory - - - Council Note the council was founded in July of 2010 with the express purpose of providing consumer input to the Strategic Plan to Prevent and End Homelessness and the secondary goals of providing ongoing meaningful Board representation to the Coalition for the Homeless from homeless and formerly homeless individuals. The CAC provides advocacy and volunteerism as well as an educational speakers series for community initiatives. 31 homeless and formerly homeless individuals participated in the founding meeting in July, 2010.
