PROGRAM SERVICE ACCOMPLISHMENT

The program works with over two dozen hospitals, law enforcement agencies across the city and most local homeless shelters and agencies. The staff has developed an understanding of agency services, requirements and capabilities. In 2011, 4,900 rides were provided to 975 individuals. These rides led to over 12,300 nights stay being arranged, 1,700 at the programs direct expense. The rides are a means to an end. They enable us to build rapport and understanding. That enables us to match the homeless' need with appropriate services and take individuals and families to appointments to address medical, legal, housing and critical needs. Providing integrated case management (assisting individuals through all steps of recovery) resulted in 193 individuals, generally chronic homeless, entering detox, 56 entering long term treatment and 53 moving into transitional housing or reintegration. When a bed is not available at the next recovery step, the program provides seamless transitions that avoids a return to the street and the opportunity for relapse. "Aged out" youth is another target population for this program. The program worked with 83 youth, helping 81% stay off the street. While most were in the program a short time, 6 experienced difficulty retaining housing. Of the 6 that were in the program most of the year, 2 received diplomas, 1 a GED and 1 entered the Job Corps. By providing transportation and helping the chronic homeless off the street, the program saved over $1,500,000 in ambulance and hospital fees, as well as police and judicial costs. In addition, matching individuals with programs where they have better odds for success and providing seamless transitions improves the efficiency of the local homeless services.
