ABBOTT HOUSE OPERATED A TOTAL OF 11 RESIDENTIAL CONGREGATE CARE (GROUP HOME) PROGRAMS FOR CHILDREN WITH SIGNIFICANT EMOTIONAL AND BEHAVIORAL PROGRAMS. THE PROGRAMS INCLUDED RAPID ASSESSMENT UNITS, DIAGNOSTIC TREATMENT CENTERS, NON-SECURE DETENTION, MENTAL HEALTH COMMUNITY BASED SERVICES, AND THERAPEUTIC GROUP HOME PROGRAMS FOR ADOLESCENTS IN CARE. THE SERVICES INCLUDED SUPPORT, STRUCTURED ENVIRONMENT, MENTAL AND MEDICAL DIAGNOSTIC SERVICES AND THERAPEUTIC INTERVENTIONS TO PREVENT PSYCHIATRIC HOSPITALIZATION OR DELINQUENCY/CRIMINAL ACTIVITY. IN THE 2011 FISCAL YEAR, CONTRACTS FOR SIX CONGREGATE CARE PROGRAMS FUNDED BY NYC ACS WERE NOT RENEWED AND THE PROGRAMS WERE DISCONTINUED. IN ADDITION, OUR RESIDENTIAL CAMPUS OPERATED TWO PROGRAMS WITH A TOTAL OF 60 BEDS FOR YOUNG MALES BETWEEN 6 AND 14 YEARS OF AGE THAT NEEDED INTENSIVE PREVENTIVE MENTAL HEALTH SERVICES. THEIR EDUCATIONAL SUPPORT WAS PROVIDED BY OUR AFFILIATED SCHOOL PROGRAM. THESE PROGRAMS WERE ALSO MAINLY FUNDED BY NYC ACS AND CONTRACTS WERE NOT RENEWED PAST THE 2011 FISCAL YEAR, WITH THE PROGRAMS DISCONTINUED AT THE END OF JUNE 2011. THE EIGHT AFFECTED PROGRAMS WERE: CAMPUS RESIDENTIAL TREATMENT CENTER (RTC) - IRVINGTON CAMPUS DIAGNOSTIC RECEPTION CENTER (DRC) - IRVINGTON DRC GROUP HOMES - EAST ELMHURST (QUEENS) & NEW ROCHELLE BELLEVUE GROUP HOMES - MANHATTAN (TWO HOMES) RAPID ASSESSMENT/HARD-TO-PLACE (HTP) GROUP HOMES - QUINCY ST (BROOKLYN) & UNIVERSITY AVE (BRONX)

ABBOTT HOUSE HAS ITS FORM 990 PREPARED BY AN OUTSIDE ACCOUNTING FIRM AND HAS ESTABLISHED THE FOLLOWING REVIEW PROCESS TO ENSURE THAT THE INFORMATION REPORTED IS COMPLETE AND ACCURATE. WHEN THE FORM 990 HAS BEEN PREPARED, REVIEWED BY MANAGEMENT AND IS READY TO BE FILED WITH THE INTERNAL REVENUE SERVICE, IT IS ELECTRONICALLY SENT TO THE AUDIT COMMITTEE OF THE BOARD OF DIRECTORS FOR ANY COMMENTS. ANY COMMENTS ARE THEN GROUPED, SUMMARIZED AND PROVIDED TO THE OUTSIDE ACCOUNTANTS. EACH ISSUE IS DOCUMENTED AND ADDRESSED UNTIL THE RETURN IS FINALIZED AND APPROVED FOR FILING. AT THE BOARD MEETING FOLLOWING THE FILING OF THE FORM 990, THE CEO AND CFO PRESENT THE FORM 990 TO THE ENTIRE BOARD. THE 990 IS ELECTRONICALLY SENT TO THE ENTIRE BOARD BEFORE THE MEETING.

A KEY EMPLOYEE, BOARD MEMBER OR OFFICER, WHO HAS AN INTEREST MUST IMMEDIATELY DISCLOSE TO THE PRESIDENT AND CEO, THE EXISTENCE AND NATURE OF HIS OR HER INTEREST IN THE PROPOSED NEGOTIATION, TRANSACTION OR ARRANGEMENT BETWEEN ABBOTT HOUSE AND ANOTHER INDIVIDUAL OR ORGANIZATION. RECORDS OF SUCH DISCLOSURE SHALL INCLUDE THE NAME OF THE INTERESTED PERSON, THE NATURE OF THE INTEREST, A RECORD OF ANY DETERMINATION MADE BY THE PRESIDENT AND CEO. ONCE THESE STEPS HAVE BEEN COMPLETED, THE CHAIRMAN OF THE BOARD PRESENTS HIS OR HER FINDINGS AND RECOMMENDATIONS TO THE EXECUTIVE COMMITTEE AND THEN PRESENTS THE COMPENSATION PROPOSAL TO THE BOARD OF DIRECTORS IN AN EXECUTIVE SESSION.
