Roane General Hospital generated gross revenue of $41,938,100 of which $16,768,692 was uncollectible due to contractual agreements with Medicare, Medicaid and other third-party payors. Bad debts due to inability or unwillingness of patients to pay amounted to $2,667,720. Additionally, charges for patients meeting 200% of the poverty level and who qualified for charity care were written off totaling $1,393,015.

As a Critical Access hospital we provided health care services to our community which included the following days of care: Skilled/ICF 12,174; Swing 1,831; and Acute Care 1,455. Additionally, we provided 25,816 outpatient diagnostic visits; 9,130 emergency room visits; 24,372 rural health clinic provider visits; and 1,379 school based clinic provider visits. Revenue and expenses related to these services are included above.

The hospital also provided community health education and health screening services at reduced fees during the year in the form of a community health fair with 307 screenings; a board of education health fair with 110 screeenings; 154 sports physicals, and 723 flu shots. Other activities include 112 heart walk participants, 730 diabetes education participants, 25 prostate clinic participants, 63 cardiac kids participants, 102 nut run/walk participants and 19 kids bike race participants. Fitness complex memberships were provided for 2,013 community members and 482 employees.

Proof of financial status is required as follows: a. If the patient is older than eighteen years old, b.     Part I, Line 7: The cost of charity care was estimated using a cost to charge ratio derived from Worksheet 2 of the Schedule H instructions.The cost of community health improvement services was derived from three factors: 1. Average hourly rate per department x employee hours. 2. Actual supply expense 3. Total charges that would have been generated if services were not "free".
    Part I, L7 Col(f): Bad debt expense amounting to $2,515,058 is included in Part IX, Line 25.
    Part II: Building Blocks is a local child care center that provides services to the community which includes several of our employees. They were unable to meet payroll and going through a reorganization and bringing in new management. Roane General's Board of Directors voted to donate the above funds to allow them operating cash to help fill their funds shortage as a one time contribution. If this facility had closed, it would have impacted staffing for Roane General Hospital since several employees would have been forced to seek other child care alternatives. This organization was the primary corporate source of child care other than in home/sitter arrangements on a one on one basis by parents.
    Part III, Line 4: Management uses collection percentages based upon historical collection experience to determine collectibility. Management also reviews troubled, aged accounts to determine collection potential. Patient accounts receivable are written off when deemed uncollectible. Recoveries of accounts previously written off are recorded as a reduction to bad debt expense when received.Bad debt expense (at cost) was determined using a cost to charge ratio derived from Worksheet 2 of the Schedule H instructions.
    Part III, Line 8: Medicare allowable costs were estimated using Worksheet B of the Schedule H instructions.
    Part III, Line 9b: SCREENING: All patients identified as self pay will be screened for Medicaid eligibility referral, provided financial counseling and processed for appropriate financial assistance. FINANCIAL ASSISTANCE: Roane General Hospital encourages patients to apply for financial assistance; patients will be screened according to eligibility rules as defined in governmental assistance program guidelines and/or Roane General Hospital's Financial Assistance Policy and Procedure. CHARITY CARE: Patients with income at or below 200% of Federal Poverty Guidelines are eligible to apply for Roane General Hospital's Charity Care program. Remaining self pay accounts will be flagged and followed up for financial education regarding their obligation to finalize a mutually acceptable financial agreement to resolve their bill.
Roane General Hospital   Part V, Section B, Line 11h: All patients are charged the same amount for services. Self Pay patient charges are automatically discounted by 20% prior to billing.
Roane General Hospital   Part V, Section B, Line 13g: Patients are referred to a financial counselor who screens them for eligibility. Also, we have a local DHHR worker assigned to the hospital show screens all self pay patients and those requesting charity for Medicaid eiligbility and assists in enrollment.
Roane General Hospital   Part V, Section B, Line 15e: Partial payment arrangements may be set up for patients unable to pay the full bill and who do not qualify for charity or other assistance.
Roane General Hospital   Part V, Section B, Line 16e: All self pay accounts are outsourced to an "Early Out" services who make phone calls, send statements and make payment arrangements before accounts are returned for Bad Debt classification at 120 days from discharge.
Roane General Hospital   Part V, Section B, Line 19d: All Self Pay patients were given an automatic 20% discount off of gross charges prior to statement being sent which adjust their charges to the average paid by all insured patients.
    Part VI, Line 2: The hospital leadership reviews and develops a strategic plan each year which sets goals for operations which includes Program Development and Refinement. The planning process for the Strategic Plan involves Executive Management which includes the Chief of Staff, VP of Financial and Support Services, VP Clinical Services, Executive Directors of all departments and the Board of Directors which is made up of community leaders. A SWAT analysis is preformed and input and discussion follows which is used to identify services that may be added, refined or deleted to meet the needs of our patients and community.
    Part VI, Line 3: Roane General Hospital will make a determination of the patient's eligibility, whenever possible, at the time service is rendered. Assistance may be requested at any time by the patient and a determination may be made after the patient is treated or at any point in the billing and collection process prior to accounts being transferred to a Bad Debt status. Signage advising patient's that financial assistance is available are posted in the Registration lobby, registration clerks inform patients, information is included in the Admission packet and all self pay patients are sent a letter informing them of the assistance and who to contact.
    Part VI, Line 4: Roane General Hospital is located in Roane County, West Virginia. The service area includes all of Roane and surrounding counties of Jackson, Calhoun, Clay and Wirt. As a Critical Access Hospital with 25 Acute Care/Swing Beds and 35 Skilled/Intermediate Care beds, we also operate three Rural Health Clinics which include Urgent Care services and three School Based Clinics. Revenue is generated from the following sources: Medicare 33%, WV Medicaid 18%, Self Pay 12%, Managed Care 17%, Other Government Payers 8% and Private Insurance 12%. Roane General Hospital is also one of the largest employers in the community with 283 employees and an annual payroll with benefits of $12,927,329. The hospital is located approx. 27 miles from both I-77 and I-79 and approx. 50 miles from the larger tertiary hospitals in Charleston and Parkersburg, WV. The nearest hospital is a small rural hospital approximately 30 miles away.
    Part VI, Line 6: The members of the Board of Directors of Roane General Hospital are all local business leaders and other community representatives. Medical Staff privileges are extended to all qualified employed physicians, consulting physicians, independent practice physicians and medical staff of the local Federally Qualified Health Center. All surplus funds are reinvested in the new medical equipment, technology or upgrades and renovation of facilities built in 1970. Specifically in 2011, funds were invested to upgrade technology for Electronic Medical Records and to meet the standards for Meaningful Use. The hospital filed for Stage 1 Meaningful Use in September 2011 and was one of only two Critical Access Hospitals in West Virginia to meet these standards of providing and tracking quality patient care. The Emergency Room is operated 24/7 and is staffed by Board Certified Emergency Medicine physicians. The hospital also operates three Rural Health Clinics, provides Urgent Care services and three School Based Clinics staffed by physicians employed by Roane General Hospital. Physicians including an orthopedist, urologist, podiatrist, and general practice/wound skin care use our clinics on specified days to screen and provide care not available in our community. Roane General Hospital's Auxiliary, operated independently from the hospital, has approx. 40 members who provide information and customer support services. They also conduct several fund raising events during the year and proceeds from their sales are donated to the hospital to offset the expense of new equipment or renovations. Roane General Hospital Foundation, Inc. was formed to allow charitable contributions to support the hospital and also as a vehicle for formal fundraising activities as required by the Hospital. This Foundation has a separate Board of Directors who oversee the operation and approve any donations to the Hospital.

  Part I, Line 6 The Organization offers an incentive compensation plan to certain members of its executive staff including the CEO, Chief of Staff, VP of Financial and Support Services, VP of Quality and Ancillary Services, VP of Nursing Services and VP of Information Services/Technology. The program is intended to provide motivation and direction for the management team to accomplish the goals and objectives set forth by the Board at the beginning of each calendar year. These goals are based upon the annual strategic plan as well as the financial performance of the hospital. Shortly after the calendar year, the management team will present its progress and accomplishments to the Board with a recommended bonus based upon the criteria set forth. The board has discretion to approve, amend, or deny any recommendations. To be eligible for the program, the executive member must serve in his/her position for at least four months and be employed at the end of the program calendar year. If the individual serves less than one year but more than four months, the incentive will be prorated.
