Explanation
Form 990, Part III, Line 2
Through 2011, significant accomplishments were noted in the quality of care provided as illustrated by Underwood's performance in the Hospital Quality Measures program. For the performance measures associated with Acute Myocardial Infarction (Heart Attack), and Heart Failure, Underwood's performance was 100% compliance across all quality indicators as of the 4th quarter of 2011, and resulted in performance consistent with the Top 10% of U.S. Hospitals as noted on Hospital Compare. For performance with Pneumonia indicators, Underwood's composite score ranked 98.3; higher than all New Jersey Reporting hospitals for the 4th quarter 2011. Efforts to reduce mortality, as measured by Healthgrades, resulted in a reduction in mortality in each of the following cohorts: Heart Failure (59%), Sepsis (54%), Pneumonia (83%), Respiratory Failure (38%), and Chronic Obstructive Pulmonary Disease (69%). Underwood also provides advanced prehospital life support services through its Paramedic (MICU) program to a tri-county service area in South Jersey. As part of our emergent primary angioplasty program for STEMI (heart attack), the Door-to-Balloon (D2B) time for patients taken to the cardiac catheterization lab averaged 58 minutes for 2011, against a national benchmark of 90 minutes. For those patients treated by the MICU, the D2B time was further reduced by an average of 20 minutes due to rapid recognition, intervention and activation of the STEMI alert prior to the patient arriving in the Emergency Department. Underwood is a member of the Jefferson Neuroscience Network and maintains certification in New Jersey as a Primary Care Stroke Center. Underwood began the process TO ACHIEVE MAGNET STATUS in 2010. THE MAGNET RECOGNITION PROGRAM WAS DEVELOPED BY THE AMERICAN NURSES CREDENTIALING CENTER (ANCC), BASED PRIMARILY ON RESEARCH CONDUCTED BY THE AMERICAN ACADEMY OF NURSING, TO RECOGNIZE HEALTHCARE ORGANIZATIONS THAT PROVIDE THE VERY BEST IN NURSING CARE AND UPHOLD THE TRADITION OF PROFESSIONAL NURSING PRACTICE. THE MAGNET RECOGNITION PROGRAM FOCUSES ON ADVANCING THREE GOALS WITHIN EACH APPLICANT AND DESIGNEE: 1: PROMOTING QUALITY IN A SETTING THAT SUPPORTS PROFESSIONAL PRACTICE. 2. IDENTIFYING EXCELLENCE IN THE DELIVERY OF NURSING SERVICES TO PATIENTS/RESIDENTs. 3. DISSEMINATING "BEST PRACTICES" IN NURSING SERVICES. WHEN A HEALTHCARE ORGANIZATION CHOOSES TO ACHIEVE MAGNET RECOGNITION, IT COMMUNICATES TO HEALTHCARE PROFESSIONALS AND THE COMMUNITY THAT IT PLACES THE HIGHEST PRIORITY ON PROVIDING QUALITY CARE FOR ITS PATIENTS AND A POSITIVE PRACTICE ENVIRONMENT FOR ITS EMPLOYEES. UNDERWOOD'S VERSION IS TO FOCUS ON AN ORGANIZATIONAL APPROACH TO CLINICAL EXCELLENCE AND PROFESSIONAL PRACTICE. The achievement of Magnet status takes several years. Underwood continued its effort to achieve this goal throughout 2011. UNDERWOOD-MEMORIAL HOSPITAL OPENED AN OUTPATIENT WOMEN'S DIAGNOSTIC CENTER IN 2009, AND SAW THE FIRST FULL YEAR OF OPERATION DURING 2010. THE CENTER OFFERS THE FOLLOWING VITAL PATIENT SERVICES TO THE COMMUNITY: FULL FIELD DIGITAL MAMMOGRAPHY, BONE DENSITOMETRY, AND VARIOUS ULTRASOUND SERVICES. THE CENTER SERVICED 167 PATIENTS DURING 2009, 1,507 in 2010, and 2,086 in 2011 a 38% increase over the first full year. During 2011 a certified breast surgeon was hired for the Center. Stereotactic Mammography was a new service added in 2011 that allows for needle guided breast biopsies. A CAT scan machine was introduced into our Emergency Room that allows us to treat patients faster, as it saves on transport time to our radiology center and reduces result time that can result in faster ER discharges. Digital Mammography was added to the main hospital campus in 2011. The outpatient catheterzation lab had an equipment upgrade in 2011. The hospital has six Family Health Centers located in the hospitals service area. During 2011 Underwood began providing outpatient laboratory services at these sites in lieu of drawing blood and sending to one of the large commercial outpatient laboratory services. The result has been that our Family Health Center physicians and patients are getting faster lab results that are automatically recorded and available in our hospital's information system. This program has proven so popular that we have begun providing outpatient laboratory services to several area nursing homes as well. Underwood took part in 34 Community Health Fairs/Events in 2011. Blood pressure screenings and blood glucose levels were among the tests performed. Area residents screened totaled 1,335 with 215 people found to have abnormal results and counseled to seek further treatment from their private physician or community health providers. Underwood centralized Same Day Surgery services in 2011 so that patients coming in for pre-admission testing could receive mostly all the needed services without traveling around the hospital to multiple departments. Existing office space was renovated to provide an area where laboratory, EKGs and physician interview can all take place. This is an important convenience for any patient with mobility limitations and saves patients the time spent waiting in multiple offices. Underwood successfully completed its triennial survey from The Joint Commission in December 2011, resulting in Accreditation with Full Standards Compliance. FORM 990, PART III, LINE 4E - OTHER PROGRAM SERVICE ACCOMPLISHMENTS: The hospital provides other outpatient services to the community. During 2011 the hospital provided outpatient adult and children psychiatric programs with an average daily census of 12.1 and 11.0 respectively. In addition, our mobile intensive care unit received a total of 27,082 calls and rendered services to 12,435 patients. The family practice center registered 349 new patients in 2011 and provided total visits of 6,290. The prenatal care program provides for expecting parents and reported 338 births and 4,393 visits in 2011. Other programs include physical/occupational therapy (18,711 treatments), speech therapy (1,321 visits), sleep studies (1,216 studies), wound care (2,670 visits), heart center treatments (3,716), and electrocardiograms (11,986). Also, the hospital provided 42,594 radiology exams, 9,273 ultrasound tests, and 17,090 outpatient CT scan tests. The off-campus outpatient family health centers provided services to support 42,594 visits in Gloucester County.
