PART I, LINE 1
ORGANIZATION'S MOST SIGNIFICANT ACTIVITY (CONTINUED)
as the demonstration that recovery from mental illness and/or addiction is possible. In order to stay on track with recovery and wellness values, THE ORGANIZATION uses a wellness based approach instead of the traditional illness based approach. The focus is on strengths and is person-centered. The following principles, "Pathways to Recovery", guide the development of all Recovery Innovations services: - Hope. Hope is the beginning. It's a thought that things can get better, a feeling of courage with a spark of new energy. Hope creates a "turning point" and means, "I can have dreams". It's a vision of a better future; learning to focus on the positive. - Choice. Recovery is a choice. Making choices is how we can discover who we are. We have the right to make choices even if it involves taking risks. This is how we learn; from our own experience. In fact, we are the experts in our own care. We do best when we choose our services, our service provider and our service plan. - Empowerment. Recovery is the person's job. We resist taking the power by doing "to" or "for". Give the person the lead role. Creating a "valued role" promotes empowerment. Promoting self-advocacy supports each person in taking the lead and not giving their power away. - Recovery Culture. Healing happens in relationships. We value and nurture relationships based on kindness and mutual respect. Maintaining a positive "level of consciousness" of compassion and caring helps to create a "community" with valued social roles for everyone and to celebrate diversity. - Spirituality: Meaning and Purpose. Connecting within and beyond the self helps move recovery forward. When we find our own answers to the mystery of spirit we develop spiritual competence. PART III, LINE 4A (CONTINUED) Recovery Response Centers and Living Room settings offer a "no force first" environment. The goal of the "no force first" environment is to provide support without seclusion, restraint, force or coercion. LINE 4C (CONTINUED) Individuals can choose classes and programs they want to help empower them to attain their personal wellness goals. Educational opportunities are developed and enhanced based on feedback received from those receiving support. Peer support programming offers a unique and powerful opportunity to engage with individuals that share lived experience. One to one opportunities, as well as groups, create recovery experiences that empower individuals to move further in their recovery journey. LINE 4D OTHER: Administrative / Management: Administration/management consists of the performance and management of operations and the implementation of major decisions for the organization, providing the universal process of organizing people and resources efficiently as to direct activities toward common goals and objectives. The elements utilized include planning, organizing, staffing, leadership, quality support and finance to meet the organization's goals. The "planning" function involves establishing goals and arranging them in a logical order in both short-range and long-term planning. "Organizing" identifies responsibilities and specifying organizational relationships, achieving coordinated efforts among all the components in the organizations. Staffing means filling job positions with the right people at the right time. Leadership guides people in a manner that achieves the goals of the organization including proper allocation of resources and providing and effective support system. Quality support evaluates quality in all areas and ensures high-quality performance. Finance implements the budget process and sets controls and processes in place.
PART VI, GOVERNANCE, MANAGEMENT AND DISCLOSURE
SECTION A, GOVERNING BODY LINE 2: The president/CEO, Eugene Johnson, has a family relationship with Lori May Ashcraft, Executive Director of Recovery Opportunity Center, LLC. LINE 4: THE ORGANIZATION'S BYLAWS WERE AMENDED TO MAKE THE ORGANIZATION'S BOARD OF DIRECTORS THE SAME BOARD FOR ALL OF THE RELATED ENTITIES. ALSO TO INCLUDE THAT ELECTION OF ANY BOARD DIRECTOR WILL BE DONE BY THE MAJORITY OF THE DIRECTORS OF EACH RESPECTIVE BOARD. SECTION B, POLICIES LINE 11: THE FORM 990 IS PREPARED BY AN OUTSIDE ACCOUNTING FIRM and reviewed by the organization's CEO and VP of Finance BEFORE FINALIZING AND FILING. LINE 12C: ANNUALLY, EACH BOARD MEMBER MUST REVIEW THE CONFLICT OF INTEREST STATEMENT AND DISCLOSE ANY POTENTIAL CONFLICT BETWEEN A BOARD MEMBER'S OBLIGATIONS TO THE ORGANIZATION AND THE BOARD MEMBER'S PERSONAL, BUSINESS OR OTHER INTERESTS. THE DISINTERESTED MEMBERS OF THE ORGANIZATION SHALL MAKE A DETERMINATION AS TO WHETHER A CONFLICT EXISTS AND WHAT SUBSEQUENT ACTION IS APPROPRIATE (IF ANY). LINE 15A & B: The organization reviewed the compensation information on the form 990 of comparable non-profits to help in determining the levels of compensation of its executive staff. The salary package was presented to the board of directors for their review and approval. The decision was noted in the minutes of the board meeting. The date of the last review cycle was August, 2010. SECTION C, DISCLOSURE LINE 19: THE ORGANIZATION MAKES ITS ARTICLES OF INCORPORATION, BY-LAWS, conflict of interest policy AND FINANCIAL STATEMENTS available TO THE PUBLIC UPON REQUEST.
