Form 990, Part VI, Line 15b: Compensation Review and Approval Process for Officers and Key Employees
According to the organizational Policy and Procedures Manual, salary reviews take into account the employee's number of years of experience, work responsibilites and performance, current market economic factors, and the organization's overall performance. Salary reviews for staff are conducted annually as part of the fiscal year budget process and employee performance review. Salary review for the executive director is conducted by the Board of Dirctors.However, since the hire of the executive director in 2005, there has only been one increase in salary for this position. The raise for the executive director was initiated and approved by the Board of Directors in 2007 but not implemented until 2009 when there were enough funds in teh budget to pay for the pay increase. As per the Policy and Procedures Manual, the pay increase was due to the increase in roles and responsibilities of the director and a review of the fiscal performance of the organization. Since this time, there have been no other raises for the executive director or any other management-level staff due to the tenuous nature of the economy and the organization's financial standing.Since the first hire of the kep personnel in 2008, there have been minimal pay increases ($1) for all key positions (Health Access Manager, Community Connection and Empowerment Coordinator, Education and Leadership Manager, Mogolian Health Access Coordinator). As per the Policy and Procedures Manual, these decisions are weighed by a review of staff performance as well as organizational funding for the year.

Per the Policy and Procedures Manual, each officer, employee, or member of the board of directors of the organization shall annually disclose any relationships, positions, or circumstances in which the person is involved that he or she believes could contribute to a Conflict of Interest. Such relationships, positions, or circumstances might include ownership of a business that might provide goods or services to the organization. Any such information regarding business interests of the person or a family member shall be treated as confidential and shall generally be made available only to the Chair of the Board, the Executive Director, and any committee appointed to address Conflicts of Interest, except to the extent additional disclosure is necessary in connection with the implementation of this Policy.

Form 990, Part III, Line 3: Ceased Conducting or Significant Changes To Services
Based on strategic planning conducted in 2011, the organization decided to integrate two of its programs (Community Connection and Empowerment and Leadership and Education) into one. The new program title is "Leadership and Empowerment Program". This was an administrative decision to bring together resources and personnel, given that both programs serve the same community. As a result, the staff of both programs now work together more closely and programs and services are more streamlined. Program services now include worker meetings and breakfasts with street outreach.
