FORM 990, PAGE 6, PART VI, LINE 11B
THE FEDERAL FORM 990 IS PREPARED AND PRESENTED TO THE EXECUTIVE DIRECTOR AND THE BOARD OF DIRECTORS AT THEIR REGULARLY SCHEDULED MONTHLY BOARD MEETING BY THE INDEPENDENT CPA. FORM 990, PAGE 6, PART VI, LINE 12C
DISCLOSURE IS DISCUSSED ANNUALLY DURING A BOARD MEETING AND WITH NEW BOARD MEMBERS AS THEY ASSUME THEIR DUTIES AND BOARD POSITIONS.
