THE REVIEW UTILIZES A PREDETERMINED FORM FOR THE SUPERVISOR AND A SELF-EVALUATION QUESTIONNAIRE FOR THE EMPLOYEE. THE SUPERVISOR'S WRITTEN ASSESSMENT OF AN EMPLOYEE IS REVIEWED BY A BOARD MEMBER PRIOR TO BEING PRESENTED TO THE STAFF PERSON AND A BOARD MEMBER OBSERVES THE REVIEW SESSION. THE ANNUAL REVIEW ALLOWS FOR A DISCUSSION OF STRENGTHS, GROWTH POTENTIAL AND SUGGESTIONS FOR FURTHER PROFESSIONAL DEVELOPMENT.
