Explanation
Form 990 Part III Section 4c TRANSPORTATION PROGRAM - Safe and afforable transportation services for residents of Fayette County. Included are local trips, out-of-town medical appointments and employment related trips outside of regular operating hours. In all, 1,271 passengers received assistance getting to and from work. The program provided 14,413 other trips which included over 11,735 trips for the elderly and handicapped. FAYETTE CO COMMISSION ON AGING NUTRITION PROGRAM - Served over 30,000 meals between the Commission on Aging facility and meals delivered to the homes of senior citizens. FAYETTE CO COMMISSION ON AGING - Transported seniors on over 2,000 trips to doctors appointments, nutrition sites, grocery shopping, and much more. Some were transported to the center so they could receive help with their Medicare, Low Income Subsidy, to receive help finding free medication and to hear educational speakers or take part in exercise programs and the congregate meal. FAYETTE PROGRESSIVE INDUSTRIES TRANSITION TEAM - Worked to transition 10 persons with disabilities into competitive employment in the community, which includes helping them get their diploma for GED, necessary clothes and shoes for work, and a reliable means of transportation. DESTINATION HOME - A housing assistance program for persons with disabilities. A total of 29 individuals were served in 22 households. Upon exiting the program, 50 had income, 50 exited to permanent housing and 100 had benefits.
Form 990 Part III Section 4c WHEELS TO WORK - Allows people to keep thier jobs, pay their bills and live independently, this program provided persons with a ride to and from work. An average of 100 people per month use this program. CARE VAN - Program Eleven for weight loss had 100 participants in the 4 month program. The Health Walkers program had 132 active participants while the Create a Motion class had 67 active participants. All groups were designed to help Fayette Countians, who rate 80th in overall health of 88 counties, lose weight and improve their health. The Care Van also did free cholesterol, blood sugar, and other health screenings to help monitor progress. HEALTH CARE HELP FUND - Programs offered assistance with 66 eye exams and eyeglasses and 55 people received prescription assistnace. HOME DELIVERED MEALS - Meals for non-seniors with special dietary needs or for people who have recently had surgery and are recovering at home. In 2012 there were 1,000 meals provided. FAYEETE CO FAMILY YMCA - In 2012, the YMCA served 387 kids in the infant to school age child care progam and an additional 114 in the summer day camp program. Day camp scholarships were provided to 21 kids at a value of 14,761. Program scholarships were provided at a value of 3,289 including 435 enrollments in swim lessons and 717 in youth sports. There were 740 individuals who benefited from YMCA scholarship assistance.
Form 990 Part III BIG BROTHERS/BIG SISTERS - Positive mentors and role models were provided to 18 kids in Fayette County. Studies show that 79 of the children with mentors had better academic performance. BOY SCOUTS - Character development and physical fitness programs for the participating boys and girls, aimed at instilling leadership abilities, self-reliance and team work. CHILDREN OFFERED RESOURCES EARLY CORE - Assessments for at-risk children form birth to age five. Once children are identified as having a delay or being at risk for a delay, therapies are provided to help the children develop on target for their age. Speech, occupational and physical therapies were provided to roughly 150 children in 2012. GIRL SCOUTS - Youth-serving organization for girls 5-17. Helps cultivate vlaues, social conscience and self-esteem while teaching cirtical life skills that enable girls to succeed as adults. In 2012 the program served 175 Fayette County girls. CORE CAR SEAT PROGRAM - Conducts car seat safety inspections to check for proper installation and offers reduced price or free car seats to those in need. JR ACHIEVEMENT - Educational programs to help students gain a better understanding of our economic and business system. The programs ties into the school curriculum and aids students in understanding these components on the state achievement tests. An Economics program was provided to every 9th grade student - 135 total. At the elementary school level, 4th grade was presented the Our Region program in 4 classes- 107 students.
