Briefly describe the organization’s mission: SHM is dedicated to promoting the highest quality care for all hospitalized patients. SHM is committed to promoting excellence in the practice of hospital medicine through education, advocacy and research from the founding days of the organization. The largest program services for SHM include Education, Quality Initiatives and Membership. SHM provides educational resources and tools, many of which are offered free to its members and the general public. SHM's annual conference is the largest gathering focused solely on hospital medicine, attracting a broad spectrum of hospital medicine professionals. It is the intention that this information will: - Improve the teaching and learning skills of physicians and the health-care team - Facilitate improved outcomes in health care - Support the maintenance of current medical knowledge and competence - Prepare hospitalists for working effectively in the hospital environment What distinguished SHM from other professional medical societies is that

EDUCATION SHM's education programs provide resources and tools to improve the teaching skills of physicians and the health care team, facilitate improved outcomes in health care, support the maintenance and update of current knowledge and competence, and prepare hospitalists for working effectively in the hospital environment. SHM focuses education and professional development programming on systems-based improvements (quality and patient safety), physician leadership, academics, and practice management. Educational objectives are based on the Hospital Medicine Core Competencies, which were published in 2006. Education is delivered through a variety of mediums, including online modules and webcasts, live meetings and print-based journals and texts. In 2012, SHM courses trained over 4,000 hospitalists with varied educational offerings. SHM hosted the largest Hospital Medicine annual conference with 2,600 attendees, seven intensive Annual Meeting Pre-courses with over 500 total attendees, and four Leadership courses. SHM also sponsored the Academic Hospitalist Academy, targeting junior hospitalists with an interest in academic medicine and serving as the leading training course for new hospitalist teachers and researchers. One of SHM's most popular courses in 2012 was the new Quality and Safety Educator's Academy, a train-the-trainer course to prepare program directors and physician educators for a new set of national training requirements for residency programs. In addition, SHM continued to develop digital learning products, offering online modules to train hospitalists in salient quality and patient safety topics. The Hospital Quality and Patient Safety Academy offers a total of 30 CME credits across 10 modules with topics like teamwork and communication, patient safety tools, organizational knowledge and leadership skills, health information systems and others. In addition, SHM developed three modules to satisfy certification board self-evaluation in medical knowledge requirements- one for the American Board of Pediatrics and two for the American Board of Internal Medicine. In 2012, SHM also committed to investing in a new learning management system, which provides necessary organizational infrastructure for additional e-learning development in 2013 and beyond.

Quality Improvements SHM recently established the Society of Hospital Medicine's Center for Hospital Innovation and Improvement (The Center) to solidify its commitment to promoting innovative and effective methods for improving patient safety and quality of care. SHM's Mentored Implementation Programs In its 2009 Annual Healthcare Quality Report, AHRQ stated that "efforts are needed to accelerate improvement." These efforts must include the implementation and dissemination of novel approaches to quality improvement. One such example is SHM's first-of-a-kind 'mentored implementation' (MI) model. The model employs a mentor - a hospitalist-expert in quality improvement and relevant content - to guide hospital teams in implementing best practices to improve quality. SHM's MI program has been underway since 2006 and encompasses four quality improvement initiatives - Project BOOST (designed to optimize patient transition from hospital to community), Venous Thromboembolism (VTE) prevention, Glycemic Control, and Hospitalist-Orthopedist Co-Management. Mentored implementation is being applied at approximately 300 hospitals around the United States, and is rapidly expanding. SHM MI program participants are in all but 6 states (see map at the conclusion of this document), as well as Canada and have received applications from Asia, South America and Latin America. The first cohort of mentors, many of whom are national experts in the area of the initiative, have come together with SHM senior staff to develop each QI initiative from the "ground up." All four of SHM's signature MI programs feature: - Regular one-on-one phone calls where the mentor works directly with the QI team - Educational webinars in which all hospitals participating in a QI initiative are invited to participate - As appropriate, face to face training between SHM staff, mentors, and participating hospitals - Email listservs, where any aspect of a QI initiative may be discussed among participating sites, mentors, and senior SHM staff from The Center - Data input and benchmarking through SHM's data repository - Access to SHM's resource room, a virtual library of resources related to a specific clinical topic - Depending upon the initiative, a one day site visit where the mentor visits personally with the hospital, its QI team, senior leadership, the medical staff and other key stakeholders SHM's Signature Mentored Implementation Programs Project BOOST www.hospitalmedicine.org/boost The national spotlight has focused attention upon the issue of preventable readmissions, its impact upon cost of care and adverse events during the post-discharge period. Unplanned rehospitalizations cost Medicare $17.4 billion in 2004. According to a study in the 2009 issue of the New England Journal of Medicine (NEJM), almost 20% of Medicare patients were readmitted within 30 days, and one-third returned within 90 days. Follow-up medical care appears to play a crucial role in these statistics, as a study discussed in a May 2010 issue of the Journal of the American Medical Association (JAMA) indicates: "Patients who are discharged from hospitals that have higher early follow-up rates have a lower risk of 30-day readmission." Now, the Affordable Care Act includes key provisions targeting readmissions, including financial penalties to hospitals for patients unexpectedly readmitted within 30 days. SHM's Project BOOST is a comprehensive program, currently implemented at 85 sites in 26 states, aimed to significantly reduce hospital readmissions by optimizing care transitions from the hospital to home. In 2002, SHM partnered with the Hartford Foundation to improve care for hospitalized older adults. Early initiatives developed included creation of a discharge planning checklist, a resource room, and implementation guide for care transitions. Based upon the success of the initial demonstration project, the Hartford Foundation awarded SHM a $1.4 million grant to launch Project BOOST. Since this time, Project BOOST has also received funding in support of the intervention from the California Healthcare Foundation, LA Care, Blue Cross Blue Shield of Michigan, and Blue Cross Blue Shield Illinois. Project BOOST sites receive: - Longitudinal Technical Assistance, providing face-to-face training and a year of expert mentoring and coaching. - The BOOST Data Center, an online resource center, allowing sites to store and benchmark data against control units and other sites as well as generate reports. - A Comprehensive Implementation Guide, providing step-by-step instructions and project management tools, such as the Teach Back Training Curriculum. - Collaboration Opportunities, allowing sites to learn from one another via the BOOST Listserv, BOOST Community website, and quarterly all-site teleconferences and webinars. Preliminary data from some sites, which implemented Project BOOST for at least 6 months, reveals a reduction in their 30 day readmission rates from 14.2% before BOOST to 11.2% after implementation; also, producing a 21% reduction in 30 day all-cause readmission rates. Pilot sites indicate that BOOST tools reduce length of stay, and improve communication and collaboration across hospital functions and outpatient physicians. Patients also perceive an increased level of service and medical attention. Project BOOST has been featured in the Washington Post, the New York Times, Health Leaders Media, and NPR. Additionally, BOOST is one of the key programs featured by the Department of Health and Human Services as a best practice for achieving the Partnership for Patients' goal of reducing readmissions by 20% by the end of 2013. VTE Prevention Collaborative www.hospitalmedicine.org/vte Venous thromboembolic disease, ranging from asymptomatic deep vein thrombosis (DVT) to massive pulmonary embolism (PE), is a significant cause of morbidity and mortality in hospitalized patients. Almost all hospitalized patients are at risk for VTE and the literature suggests that approximately half of all VTE's are hospital-acquired. PE is recognized as the cause of death for more than 100,000 hospitalized patients in the United States every year, and is considered a contributing factor in the death of 100,000 more patients. Hospitalists are ideally positioned to reduce the incidence of preventable VTE's, both by using known best-practices to improve care delivered to their own patients, and, more importantly, by leading hospital-wide QI efforts that improve care for all patients at their home institutions. In 2006, SHM launched the VTE prevention collaborative where SHM Mentors with QI and VTE expertise work with hospitalists who wish to lead local VTE prevention efforts. 77 hospitals have or are currently participating in this initiative. As of June 2010, outcomes for program participants were as follows: - At baseline only 14% had a prophylaxis rate of 60% or better; after participation, this figure rose to 100% - 74% achieved rates above 80% - 40% of sites are now operating at hierarchy of reliability level 5, where outliers are identified and addressed in real time (the gold standard for QI). SHM's VTE program is one of the key programs featured by the Department of Health and Human Services as a best practice for achieving the Partnership for Patients' goal of reducing hospital acquired conditions by 40% by the end of 2013. Glycemic Control www.hospitalmedicine.org/gcmi Diabetes mellitus (DM) has reached epidemic proportions in the United States. A reported 9.3% of adults over 20 years of age have diabetes, representing over 20 million persons. Despite increasing awareness, diabetes remains undiagnosed in approximately 30% of these persons.3 Concurrent with the increasing prevalence of diabetes in the U.S. population from 1980 through 2003, the number of hospital discharges with diabetes as any listed diagnosis more than doubled, going from 2.2 to 5.1 million discharges.4 Hospital care for patients with diabetes and hyperglycemia poses a significant health economic burden in the United States, representing over 40 billion dollars in annual direct medical expenditures.5 SHM convened a Glycemic Control Task Force in 2005, with the intent of improving the care of inpatients with diabetes. The goal of the Task Force was to provide hospitalists and quality improvement teams with an understanding of the best practices to achieve safe glycemic control in the hospital. The Task Force ultimately led to the development of a web-based compendium of tested tools and strategies to assist local improvement teams, as well as a technical assistance program designed to provide physician coaching with expertise in glycemic control to these teams. Today, over 100 hospitals participate in SHM's Glycemic Control Mentored Implementation program. Associated with this program is a robust data registry that allows providers an easy way to track and trend data associated with glycemic control. Additionally, the program features an active commun

Membership SHM is dedicated to promoting the highest quality of care for all hospitalized patients. The organization supports and enhances the practice of hospitalists and the patients they serve through a network of almost 12,000 members. SHM provides its members with a range ofpublications and information , including but not limited to: The Hospitalist (published monthly), The Journal of Hospital Medicine (9x per annum), various newsletters such as The Hospitalist e-wire (weekly), SHM's e-wire (bi-weekly), InfoPoems (2x per month), e-TOCs (monthly). . SHM has also developed a range of products and services to promote quality improvement and practice management efficiency. SHM provides its members with a variety of tools and services that facilitate communication and networking. Through its website, members can participate in several collaborative online communities focused on topics of interest to hospitalists. Through local SHM chapters, members can meet their peers and hear presentations from experts in the field. Members have access to SHM's Legislative Action Center, online educational resource rooms, core competencies and discounted reference materials and educational seminars. All requests for discounted membership fees or attendance fees at meetings on the basis of financial need are considered by management on a case by case basis. In 2009, SHM created a program for Physician hospitalists who are devoted to hospital medicine and whose personal and professional activities embody both the mission and goals of the Society as well as the principles and responsibilities of medical professionalism. In 2012, this program was expanded to include additional members of the healthcare delivery team; specifically- Nurse Practitioners, Physician Assistants and Hospital Medicine Practice Administrators. Becoming a Fellow is an avenue for special recognition of SHM members who have distinguished themselves amongst their colleagues through: Professional Development, Lifelong Learning, Dedication to quality and process improvement and Commitment to organizational teamwork and leadership.

SCHEDULE D, PART X, LINE 2   The Internal Revenue Service ("IRS") has classified SHM as exempt from federal income tax under Section 501(c)(3) of the Internal Revenue Code ("Code"); SHM qualifies for the charitable contribution deduction under Section 170(b)(1)(A) of the Code, and has been classified as an organization other than a private foundation as defined in Section 509(a)(2) of the Code. Accounting for Uncertainty in Income Taxes clarifies the accounting for uncertainty in income taxes recognized in an enterprise's financial statements. Management has analyzed the tax positions taken by the Organization, and has concluded that as of June 30, 2012, there are no uncertain positions taken or expected to be taken that would require recognition of a liability or disclosure in the financial statements. The Organization recognizes accrued interest and penalties associated with uncertain tax positions, if any. There was no income tax related interest or penalties recorded for the years ended June 30, 2012 and 2011. The income tax returns of the Organization for the years ended June 30, 2009, 2010 and 2011 are subject to examination by the IRS and other taxing authorities, generally for three years after they were filed.

SCHEDULE I PART I, LINE 1   Applicants for SHMs grant awards are required to submit a budget with their proposals detailing how the funds will be used. A review panel comprising experts from SHMs Academic and Research Committees ensures that the applications selected for the award meet the stated eligibility criteria, and that their proposed use of funds meets the grants requirements.
SCHEDULE I, PART I, LINE 2   Grant recipients are required to provide a progress report within 6 months of receiving their acceptance notification describing their achievement of the milestones outlined in the proposal. They are asked to submit any requests for changes to the use of funds as described in the proposal in writing to SHM, for the consideration of the Academic and Research Committees.
