		Client Note 1 - Attachment to Form 990The trust was formed on May 1, 2009 for the sole purpose of funding health and dental benefits for Senior Care Management, LLC and affiliates. Prior to filing the return, the year end was changed from December 31 to April 30. Due to the change in the plan year end and other factors related to the initial year, the extension for the initial plan year due September 15, 2010 was inadvertantly not filed. In addition, the determination letter which stated the requirement for filing Form 990 was not received until November 9, 2010.
