Sinai Health System's "Helping Her Live" progam aims to empower women with knowledge,help women who are 40 years and over obtain annual screenings and work with women and health care facilities to eliminate delays in obtaining test results and ensuring follow up as necessary.With Support from the Avon Foundation for Women,our Program assists women with scheduling appointments, navigating through the hospital to appointments, assisting to overcome barriers with such matters as transportation, social support, referrals to support groups, follow-up on all breast cancer diagnoses and treatment planning. The Greater Humboldt Park Community Diabetes Empowerment Center at 2753 West Division Street, Chicago is a community-driven partnership of five medical providers (Sinai Health Systems, Northwestern Medicine, Norwegian American Hospital, Saints Mary and Elizabeth Medical Center, and Rush University Medical Center) and three community organizations (The Greater Humboldt Park Community of Wellness, The Puerto Rican Cultural Center, and Pueblo Sin Fronteras). The Center is a highly visible community resource to prevent and control Type 2 diabetes for the residents of Greater Humboldt Park and surrounding neighborhoods. Since Type 2 diabetes is impacted by both physiological and social determinants of health, the Center is part of a multi-level community-driven approach that provides health education, physical activities, and social support to neighborhood residents in a community environment that facilitates collaborative learning about diabetes. The Center strives to inform, advocate, and empower residents regardless of literacy, language, insurance status, or income to reduce the health risks associated with diabetes. The Center houses the staffs of the NIH funded Block-By-Block. The Greater Humboldt Park Community Campaign Against Diabetes of which Sinai Health System is also a partner helps to reduce the burden of diabetes and childhood obesity in the communities of North Lawndale (NL) and South Lawndale (SL), located on the west side of Chicago.

Other Programs include the Stand Against Cancer Evaluation, Access Community Health Network (ACCESS) has implemented the Stand Against Cancer (SAC) program in partnership with the state of Illinois in order to address disparities in breast and cervical cancer outcomes by targeting low income, minority, and uninsured women in the Chicago area. As a partner in this effort, SUHI serves as an external evaluation consultant, and is responsible for data cleaning, analysis, and the creation of the final evaluation reports needed to monitor trends and progress towards program objectives.The community health worker programs primary aim is to clearly delineate community health worker (CHW) effectiveness and best practices in terms of evidence-based science. Information collected in the study will be synthesized and analyzed to create standardized process and outcome evaluation tools for CHW programs.The project has completed an extensive literature review on CHWs.Sixty two CHWs and twnety one administrators of local CHW programs in health care settings were surveyed on a wide range of topics, including CHW titles, definition, roles and job duties, training, supervision, hiring and compensation, as well as CHW program funding and evaluation. Preliminary project findings have been compiled and reported back to community partners and the project's funder. The research has been presented at the national American Public Health Association's Annual Conference in October 2012 and the local 2013 CHW Forum. The project is working on compiling CHW best practice guidelines in CHW hiring, training, supervision and CHW integration into health care settings, as well as creating standardized CHW evaluation tools. All findings will be available to the public. Provisions of the Affordable Care Act, such as hospital readmission reduction and patient-centered medical homes, will match CHW roles and work. These guidelines may assist health care agencies with new uptake of the CHW model and its principles. Ultimately the intention is that the work done on this project will lead to better implementation of the CHW model, better training, higher standards of evaluation, improved patient care, improved community connection with the health care system, and ultimately improve health outcomes and quality of life for vulnerable communities. The Chicago Community Trust Evaluation's purpose is to provide an evaluation framework for Trust-funded health programs, thereby allowing for critical program review by both grantee organizations and the Trust. This project was conceptualized because many community-based health programs funded by the Trust do not have a structured evaluation plan in place, making it difficult to discern program effectiveness. The SUHI evaluator has worked with fourteen CCT health grantees over the past 2.5 years, including the following current community partners: Enlace,TCA Health,Chicago Public Schools,Gary Comer Youth Center,Illinois African American Coalition for Prevention and the greater Humboldt Park Community in Motion.Although most of the partner grantees have health and wellness interventions, each program focuses on a unique population and has required distinct technical and evaluation support. Tailored program assistance, including curriculum recommendations, the restructuring of program outcomes, data collection tool development, and database building, has been provided to each organization as needed. SUHI has also held quarterly meetings with grantees to provide evaluation training and allow for input and collaboration in a group setting. The topics of these meetings have included the importance of outcomes-based evaluation, steps for developing high quality programs and evaluation plans, and guidelines for collecting and analyzing data. SUHI would like to create sustainable improvements to the evaluation process at the Trust. Ultimately, our goal is to empower each partner organization to take responsibility for their own evaluation so they are able to carry on these activities without continued support from SUHI. We hope this project will shift evaluation planning to the forefront and ensure that grantee organizations will take responsibility for their program evaluation, thereby creating a sustainable evaluation structure for the Trust and its grantees.

Form 990, Part VI, Line 15b Form 990, Part VI, Line 15b: Compensation Review and Approval Process for Officers and Key Employees Sinai Health System has an Executive Compensation Committee to support the organizational performance of Sinai Health System and its related entities through the alignment of executive compensation with system strategies and programs and ensure compliance with applicable law. The voting members of the Committee are independent members of the Board of Directors. The Committee meets twice annually, or more frequently,as circumstances require. The Committee establishes detailed goals annually for the President and CEO and other executives, and reviews performance against these goals on an annual basis. The Committee annually engages an outside, independent compensation consultant to benchmark the salaries and benefits of the organization's Assistant Vice Presidents and above, as well as a few directors. Compensation is based on detailed written performance appraisals and external market data. In executive session, the Committee reviews the performance of the President and CEO, each element of compensation, data of compensation programs in effect for CEOs of comparable organizations, and conducts an annual review of CEO performance against established goals. The Committee maintains written minutes which are maintained in Executive Administration.
Form 990, Part VI, Line 12c Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of Conflicts The Conflict of Interest Disclosure Form is completed and signed annually by all board members, employed physicians, management personnel as well as other employees who are in a position to influence purchasing decisions. In addition to the annual filing of the form, an update is required to be filed any time there has been a change. Based on the information disclosed, the Chief Integrity Officer, along with other members of management and counsel as needed, implements an appropriate conflict management plan, which may include requiring the interested party to abstain from participation in certain decisions. The information disclosed also is taken into consideration in making Board and Board Committee assignments.Further, board members are required to disclose potential conflicts relating to Board and Committee proceedings to the appropriate Board or Committee Chair for appropriate conflict management procedures, including abstention from participation in certain decisions.
Form 990, Part VI, Line 11 Form 990, Part VI, Line 11: Form 990 Review Process The 990 was initially prepared by the Finance Staff with multi disciplinary inputfrom Public Affairs, Corporate Integrity and other appropriate staff of theorganization. The 990 was then reviewed by senior finance staff and other members ofSenior Leadership. Ernst & Young,LLP reviewed the return for the organization. Priorto the filing, the 990 was reviewed and discussed with the Executive Committee ofthe Board of Directors. The 990 was also made available to the full Board ofDirectors.
Form 990, Part III, Line 4d Form 990, Part III, Line 4d : Other Program Services Description OTHER PROGRAM SERVICES 4: Support from the Aids Foundation of Chicago allows Sinai to provide Primary and HIV medical specialty care to 85 uninsured clients in our Adult Infectious Disease clinic. Throuh the clinic, Sinai caregivers provide medical and social support case management services to all qualified HIV infected patients. This includes food vouchers, transportation back and forth to visits, support systems, linkage into care, and referrals sources.Case manager case load is approximately 170 patients. OTHER PROGRAM SERVICES 5: The Michael Reese Health Trust Program's purpose is providing leadership and evaluation support to a MRHT-funded project entitled "Activating Deaf Health Care Consumers and Health Care Organizations to Create Deaf Friendly Health and Mental Health Care", a collaborative project of Sinai Health System and Advocate Health Care (the project itself is funded via a separate grant). In partnership with a number of Deaf professionals working at social service agencies in the Chicago area, this project developed, implemented and evaluated a Patient Activation group education program in American Sign Language for Deaf adults. Two hundred fourteen people officially enrolled in one of the 30 classes offered over a two year interval. SUHI personnel funded by the grant have directed, coordinated and facilitated the implementation of all evaluation activities, and have contributed extensively to reporting and dissemination.The program also works to improve respiratory outcomes and to minimize emergency department use and readmissions due to respiratory issues. The first step is to better understand how respiratory conditions such as asthma and COPD affect the communities surrounding Sinai. A detailed analysis of mortality and hospitalization rates in Chicago with particular attention to disparities by race,ethnicity and geography was completed in 2012. Significant strides were also made in continuing to expand Sinai's proven Community Health Worker Pediatric Asthma Program to new populations and in establishing relationships with funders for such expansions.
