Sch L, Part IV		Geoffrey Post: Mr. Post is a shareholder, director and
		President of Citizens State Bank (CSB). As of 12/31/2012, the
		organization had placed deposits of $4,535,885 at CSB.
		$3,514,959 of these funds were participating in CDARS
		(Certificate of Deposit Account Registry Service) through
		an agreement with CSB. The organization paid Mr. Post $300 for his
		attendance at board meetings in 2012. Total transactions
		exceed $4.536 million.
Sch L, Part IV		Henry Lodge: Mr. Lodge is a shareholder and director of
		Citizens State Bank. (See Geoffrey Post -above- for detail
		of transactions with CSB). Mr Lodge is also an officer
		and shareholder of an organization which paid disposal fees
		of $1,587 to the organization in 2012. In addition, Mr.
		Lodge received $300 for his attendance at board meetings.
		Total transactions exceed $4.536 million.

		organization from Marion County, TN. All payroll taxes
		insurance & benefits paid by the county are fully reimbursed by the
		corporation. Most employees receive a W-2 from Marion County
		for their regular payroll (as shown in column E) and a W-2 from
		the organization for bonuses, etc.
