FORM 990, PART III
The medical services program includes medical services provided to youths in the organization's programs who are in need of medical attention. medical services include, assessment, diagnosis, treatment, and/or referral at our on-site health clinics. The youth made a total of 2,518 clinic visits in Fort Lauderdale. The outreach program is an effort to reach youths who would otherwise not find their way to the shelters. outreach workers, who travel by van and on foot to meet with homeless and at risk youth at places where they congregate, made 4,228 significant contacts with youths on the streets (3,282 in Fort Lauderdale and 946 in Orlando). The community service centers program provides comprehensive services to youths who have left the organization's crisis centers and to other youths in the community who need support to maintain stable living situations. An average of 4 individual youths sought walk-in services at the community service center at the Orlando shelter. A total of 343 unduplicated youths visited 1,888 times requesting food, shower, clothing, case management, counseling, referrals, employment, life-skills instruction, and other skill-building sessions. this included, 25,915 counseling and case management sessions, 2,291 days of residental substance abuse treatment and intervention, 456 GED sessions, and 1,020 life skills training sessions provided in Fort Lauderdale. In addition, a total of 350 youths attended 262 job training sessions in fort lauderdale. The public education program informs and educates the public on how to identify potential "runaway" and "throwaway" adolescents, the public and private resources available to help such adolescents before they leave home and the public support services available to these families to improve their home environment.

Pursuant to Covenant House Florida's organizing documents, the right to approve all nominees to the Board of Directors has been reserved to its parent organization, Covenant House, Inc. This represents the only decision of the governing body that is subject to approval.

The form 990 was prepared by an independent accounting firm and reviewed by the Director of Business Management Services and the Covenant House Florida Board Finance committee for completion and accuracy prior to filing. In addition, the Finance department of Covenant House Inc. reviews the 990 prior to submission.

Conflict of interest policy enforcement and monitoring
The conflict of interest forms are filed annually by the Board and selected staff. The Covenant House Florida Board reviews all questionnaires and recognizes any conflict of interest reported. If any conflicts are reported, those individuals are removed from the process of review and approvals of any business dealings reflected in the conflict forms. This review by the board of directors is reflected in the Board meeting minutes annually.

In addition to the survey, The board finance committee reviews the pay scale on an annual basis.
