FILED FORM 1023, APPLICATION FOR RECOGNITION OF EXEMPTION UNDER SECTION 501(C)(3) OF THE INTERNAL REVENUE CODE, PART X, PUBLIC CHARITY STATUS, LINE G - (509(A)(1) AND 170(B)(1)(A)(VI) - AN ORGANIZATION THAT RECEIVES A SUBSTANTIAL PART OF ITS FINANCIAL SUPPORT IN THE FORM OF CONTRIBUTIONS FROM PUBLICLY SUPPORTED ORGANIZATIONS, FROM A GOVERNMENTAL UNIT, OR FROM THE GENERAL PUBLIC WAS MISTAKENLY CHECKED. IN FISCAL YEAR 2012, WE EXPANDED OUR SERVICES TO INCLUDE OFFICES IN JOHNSON CITY, TENNESSEE, AS WELL AS BOLSTERED CARE IN RURAL AREAS OF VIRGINIA. IN FISCAL YEAR 2012, WE HAD 25,119 OFFICE VISITS AND 16,318 HOSPITAL/NURSING HOME VISITS. REGARDLESS OF RACE, RELIGION, ETHNICITY OR ABILITY TO PAY, WELLMONT MEDICAL ASSOCIATES, INC. TREATS ALL PATIENTS FOR MEDICALLY NECESSARY CONDITIONS. RECOGNIZING THAT SOME PATIENTS CANNOT AFFORD ESSENTIAL MEDICAL SERVICES, WE PROVIDED CARE FOR INDIGENT PATIENTS INCURRING COSTS OF 59,924. ADDITIONALLY, WE PROVIDED CARE TO PERSONS COVERED BY MEDICARE, MEDICAID, TENNCARE AND CHAMPUS AT LESS THAN COST. DUE TO MANDATED REDUCTIONS IN MEDICARE FUNDING FROM THE BALANCED BUDGET ACT AND DECLINING REIMBURSEMENT FROM TENNCARE, THE SHORTFALL BETWEEN RECEIPTS AND THE COST OF PROVIDING CARE TO THESE PATIENTS TOTALED 18,125,650. OTHER COMMUNITY ACTIVITIES TOTALED 11,568. OVERALL, WE RETURNED MORE THAN 18,197,142 IN BENEFITS TO OUR REGION. IT IS OUR OBLIGATION TO SUPPORT THE CAUSES THAT IMPROVE THE QUALITY OF LIFE IN OUR REGION. WELLMONT MEDICAL ASSOCIATES, INC.'S FINANCIAL STATEMENTS ARE INCLUDED IN THE CONSOLIDATED FINANCIAL STATEMENTS OF WELLMONT HEALTH SYSTEM. WELLMONT MEDICAL ASSOCIATES, INC.'S FINANCIAL STATEMENTS FOR THE YEAR ENDED JUNE 30, 2012 WERE AUDITED AS PART OF WELLMONT HEALTH SYSTEM'S CONSOLIDATED FINANCIAL STATEMENTS. WELLMONT HEALTH SYSTEM HAS AN AUDIT COMMITTEE WHICH ASSUMES RESPONSIBILITY FOR OVERSIGHT OF THE AUDIT OF ITS FINANCIAL STATEMENTS AND THE SELECTION OF AN INDEPENDENT AUDITOR. THIS REVIEW PROCESS HAS NOT CHANGED FROM THE PRIOR YEAR.
