What is the organization's primary exempt purpose? The mission of the Raleigh/Wake Partnership to End and Prevent Homelessness is to reorient the service system from one that manages homelessness to one that prevents and ends homelessness through the power of partnerships. It does this by developing and implementing the 10 Year Plan to End and Prevent Homelessness. The Partnership is comprised of non-profit agencies that serve the homeless, as well as faith communities, government entities, private individuals and concerned businesses and corporations.
Describe the organization’s program service accomplishments for each of its three largest program services, as measured by expenses. In a clear and concise manner, describe the services provided, the number of persons benefited, and other relevant information for each program title.
28 The Raleigh Wake Partnership to End and Prevent Homelessness had the following accomplishments in FY 2011-12. The Partnership coordinated 2 major grant applications to the Department of Housing and Urban Development for programs and services for people experiencing homelessness in Wake County, NC. The HUD NOFA funding and the Emergency Solutions Grant brought more than $2.5 million dollars into Wake County to house and serve people who were homeless. The Partnership coordinated the 100,000 Homes Campaign which was an extensive effort to identify the 100 most vulnerable homeless individuals in order to better coordinate services to enable them to move into housing. The Partnership initiated a pilot program for centralized intake, information and referral services for women and children experiencing homelessness. The program was begun through a partnership between PLM Families Together and the Women's Center of Wake County. The Partnership coordinated Project Homeless Connect, a day-long event where 25+ service providers gathered in one location to provide services to over 300 homeless individuals.

29 The Support Circle Program for Homeless Families moved 15 adults and 39 children out of homeless shelters and into permanent housing and provided them with a year-long rental subsidy and the support of a team of 6 to 10 volunteers for each household. Another 8 adults and 14 children were served during the year. All successfully completed the program while maintaining their permanent housing for at least a year. The Partnership to End and Prevent Homelessness provided grants to CASA, Inc. to cover a third of the rental subsidies for the families participating in the program. At the behest of the Partnership the program is coordinated by Catholic Charities Raleigh Regional Office. Catholic Charities recruits churches to form Support Circle teams of 6 to 10 volunteers. After these teams are trained by Catholic Charities they are matched with families referred by homeless shelters which are partner agencies of the Partnership to End and Prevent Homelessness. These Support Circles provide assistance tailored to the needs of each family. The assistance may include help finding or improving employment; securing medical and dental care; budget counseling; emergency financial assistance; help procuring an affordable car; tutoring of children; and more. There were no program service expenses incurred by the Partnership for this program as these expenses were incurred by Catholic Charities. The grant money was paid to CASA, Inc. for a portion of the rental subsidies.

F99Z_P01_S00_L10 Form 990-EZ, Part I, Line 10 The Support Circle Program for Homeless Families recruited and trained churches to form Support Circles teams of 6 to 10 volunteers. These Circles were then matched with homeless families and individuals living in homeless shelters. The Circles mentored the families and provided assistance as the families transitioned from homelessness into permanent housing. One element of the program is a rental subsidy provided to participating client households. Grants were made from the Raleigh Wake Partnership to End and Prevent Homelessness to CASA, Inc. to pay a portion of the rental subsidies for families participating in the program.
