15a) The Executive Committee is responsible for working with the CEO in creating a comprehensive evaluation tool that is reviewed by the Chairman and Executive Committee. The Executive Committee monitors the performance of the CEO throughtout the year and the CEO formally reports on the status of goals at the full board meetings. In NOvember of each year, the CEO is required to submit a detailed report to the Execuitve Committee which highlights the accomplishments made in each performance standard. Each member of the Executive Committee individually reviews this report and completes a rating sheet that is submitted to the Chairman. The Chariman is responsible for conducting a meeting with the Executive Committee without management present. In December of each year, the Executive Committee meets with the CEO to review the overall performance of the CEO and recommends areas for improvements.15b) The senior management team of the organization, including all employees, review and participate in a formal performance evaluation process with their immediate supervisor. An annual written evaluation is completed for every employee and a face to face meeting is conducted with the supervisor and employee to discuss accomplishments, challenges and an overall performance rating is given. These performance evaluations are signed by the employee and supervisor and filed in a locked personnel cabinet.As a member of the Serious Fun Children's Network, we acquire and review comparability data for all positions for the purpose of creating a competitive work environment.

On an annual basis, every Board Member is required to review and sign off on a conflict of interest policy and submit it to the CEO at the annual Board Meeting. The CEO is responsible for compiling all information pertinent to the compliance of the Board regarding Board Criteria requirements. This information is shared with the Chairman and governance committee each year and based on the information received, the governance committee makes formal recommendations to the chairman and nominating committee.
