AT THE END OF THE YEAR WE HAD THE ED COMPLETE A SELF ASSESSMENT OF RESULTS RELATIVE TO THE GOALS. 3. A SUBGROUP (REVIEW COMMITTEE) OF THE COMPENSATION COMMITTEE PREPARED THE PERFORMANCE CRITERIA TO BE USED IN THE REVIEW. 4. THE PERFORMANCE EVALUATION FORMS WERE PROVIDED TO ALL CHAPTER BOARD MEMBERS AND DIRECT REPORTS. THEY WERE COMPLETED AND RETURNED DIRECTLY TO THE COMPENSATION COMMITTEE. 5. THE COMPENSATION COMMITTEE COMPILED THE QUANTITATIVE RESULTS AND SUMMARIZED THE QUALITATIVE COMMENTS. 6. THE BOARD PERFORMANCE REVIEW RESULTS AND ED'S GOALS AND SELF ASSESSMENT WERE REVIEWED WITH THE COMPENSATION COMMITTEE. RECOMMENDATIONS FOR KEY COMMUNICATION POINTS AND INPUT INTO FUTURE GOALS WERE AGREED UPON. 7. THE COMPENSATION COMMITTEE DISCUSSED COMPENSATION SCENARIOS AND CAME TO A RECOMMENDATION. 8. PROGRAMS AND SERVICES
The Minnesota-North Dakota Chapter provided more than 42,000 individuals with dementia, their care partners, community members and health care professionals with programs and services during the year ended June 30, 2013. Education and Support 24/7 Information Helpline. More than 6,300 calls were made to the Helpline for information, support and resources during the year ended June 30, 2013. Community and Professional Education The Association offered dementia-specific workshops and conferences throughout the two-state area. The Meeting of the Minds: Dementia Conference 2013 had 1,316 participants and, in total, more than 20,100 people attended community or professional education programs during the year ended June 30, 2013. Support Groups 128 active support groups, led by trained volunteer facilitators, provided assistance and encouragement to individuals and their care partners affected by dementia during the year ended June 30, 2013. Care Consultation 1,480 individuals with memory loss and their care partners received 2,164 care consultations, giving them individualized assistance with problem solving, planning and identifying resources during the year ended June 30, 2013. MedicAlert + Alzheimer's Association Safe Return During the year ended June 30, 2013, 77 people enrolled in this 24-hour, nationwide, emergency response service for individuals with dementia who wander or have a medical emergency. In total, 2,935 people are enrolled across Minnesota and North Dakota. Website The Alzheimer's Association website contains a wealth of information regarding Alzheimer's and related dementias and had more than 78,600 web visits during the year ended June 30, 2013. Advocacy In FY13, Alzheimer's Association staff and constituents initiated and advocated for the Minnesota Caregiver Sick Leave bill, which mandates that employers who provide sick leave must extend this benefit to caregivers of those with Alzheimer's disease. In May 2013, the legislature passed it. In April, 34 advocates traveled to Washington D.C. for the annual Alzheimer's Association Advocacy Forum, where they requested additional funding for Alzheimer's research and support of the National Alzheimer's Project Plan. Research Nationally, the Alzheimer's Association is a leader in the global fight for a world without Alzheimer's. The Alzheimer's Association leads in many ways, including through an international research grants program and an annual, international conference for researchers and professionals. The Association also offers TrialMatch an opportunity for the general public to be leaders by participating in clinical trials. Volunteers The Association's programs, services and events would not be possible without the commitment and generous support of volunteers. During the year ended June 30, 2013, more than 600 volunteers provided more than 10,900 hours of service.
