Identifier Return Reference Explanation
Pt VI, Line 6   Charleroi Co-Op has members, both regular and social, but it does not
    have any stock holders. Its membership varies from year to year,
    usually between 575 and 675 members. Only its regular members
    can vote (social members cannot vote). All of its officers
    are regular members.
Pt VI, Line 7a   All of Charleroi Co-Op's officers (members of its governing body) are
    elected by secret ballot by its members and the person receiving
    the majority of the votes will be declared elected for a one year term.
Pt VI, Line 7b   The only decisions of the governing body that are subject to the
    approval of the members are: (1) if a member has been suspended by the
    governing body for conduct unbecoming a member, that member may take an appeal
    of the governing body to the Lodge (members at large), provided the appeal
    is presented in duplicate form to the Secretary within 15 days
    from the time the member receives the suspension notice. The decision of the
    members at large is final; and (2) members may amend the by-laws or any part thereof
    (which by-laws were originally approved of by the governing body) by
    presenting a resolution in writing to the Secretary, who will then
    present the resolution to be read at the next two (2) meetings and
    to be voted on at the third meeting. To pass, the resolution must have
    2/3 vote of all members present at the third meeting.
Pt VI, Line 11b   The form 990 is reviewed by the President and the Treasurer before
    being remitted to the IRS. No other officers or members review the return
    prior to filing. However, the Co-Op's exact copy of the filed
    return is kept at the Co-Op's address, at 805 Meadow Avenue,
    Charleroi, PA 15022, with the Treasurer, George Maund. Any
    member of the governing body may review the copy of the return

    at any time.
Pt VI, Line 15a   Not applicable- none of the Co-Op's CEO, Executive Director,
    top management officials, other officers or key employees are
    paid for any work they provide the Co-Op. These are all volunteer positions.
