An extension request was submitted and approved by the IRS prior to the initial due date of this return. The person compiling this form is a volunteer and has a full-time paid job elsewhere. It was difficult to complete the form within the due date. In addition the web site used to create the filing was down for maintenance a few times when I had time to work on the return, which caused me to be later than I would have otherwise been.
