Form 990, Part VI, Line 4: Description of Significant Changes to Organizational Documents	AT THE JUNE 2013 CONVENTION CHANGES TO THE BYLAWS WERE VOTED ON AND PASSED BY THE MEMBERS. THESE CHANGES ARE TO GO INTO EFFECT IN 2014 AND RELATE TO THE BUDGET AND HOW FUNDS ARE TO BE DISBURSED.
Form 990, Part VI, Line 7b: Describe Decisions of Governing Body Approval by Members or Shareholders	Changes to the bylaws are voted on by the members. Members are also responsible for voting into office all officers and trustees at their convention every year.
Form 990, Part VI, Line 11b: Form 990 Review Process	Prior to the filing of the return, a draft copy is sent to the Treasurer, Secretary, and Financial Secretary. Upon their approval, the return is filed and copies given to all members of the governing body.
Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of Conflicts	Conflicts of interest are addressed in the Organization's bylaws and membership application. Members must list possible conflicts on their membership application, and if they occur, they are denied membership. The Organization is reviewing the written conflict of interest policy recommended by the IRS.
