CORE FORM, PART III; STATEMENT OF PROGRAM SERVICE ACCOMPLISHMENTS Background ========== Monmouth Medical Center ("MMC") is a general medical and surgical hospital. MMC is recognized by the IRS as an internal revenue code section 501(c)(3) tax-exempt organization. Pursuant to its charitable purposes, MMC provides medically necessary healthcare services to all individuals in a non-discriminatory manner regardless of race, color, creed, sex, national origin, religion or ability to pay. Moreover, MMC operates consistently with the following criteria outlined in IRS revenue ruling 69-545: 1. MMC provides medically necessary healthcare services to all individuals regardless of ability to pay, including charity care, self-pay, Medicare and Medicaid patients; 2. MMC operates an active emergency department for all persons which is open 24 hours a day, 7 days a week, 365 days per year; 3. MMC maintains an open medical staff, with privileges available to all qualified physicians; 4. Control of MMC rests with its board of trustees and the board of trustees of Barnabas Health, Inc. Both boards are comprised of independent civic leaders and other prominent members of the community; and 5. Surplus funds are used to improve the quality of patient care, expand and renovate facilities and advance medical care, programs and activities. The operations of MMC, as shown through the factors outlined above and other information contained herein, clearly demonstrate the hospital provides substantial community benefit and that the use and control of MMC is for the benefit of the public and that no part of the income or net earnings of the organization inures to the benefit of any private individual nor is any private interest being served other than incidentally. MMC's sole corporate member is Barnabas Health, Inc. ("BH"), a tax-exempt New Jersey non-profit parent corporation. Barnabas Health is a tax-exempt integrated healthcare delivery system consisting of a group of affiliated healthcare organizations. This integrated network of healthcare providers throughout the state of New Jersey provides substantial community benefit. The sole member or stockholder of each entity is either BH or another BH affiliate controlled or owned by BH. MMC, like its corporate parent is a not-for-profit organization. It is the oldest and largest of the centers in New Jersey. - MMC has received full accreditation as a cycle III accredited chest pain center from the Society of Chest Pain Centers ("SCPC"). Cycle III is the highest level of accreditation. - MMC received CT accreditation from the American College of Radiology ("ACR") with zero deficiencies. The Leon Hess Cancer Center at Monmouth brings together a host of specialists in a convenient setting to offer comprehensive cancer services. MMC is accredited by the Commission on Cancer of the American College of Surgeons as a "teaching hospital cancer center" - the group's highest designation. MMC is a part of The Penn Cancer Network - a select group of community hospitals in New Jersey, Pennsylvania and Delaware - collaborating with the Abramson Cancer Center at the University of Pennsylvania to provide excellence in patient care throughout the region. The Cranmer Ambulatory Surgery Center The center provides a full spectrum of same-day surgical services using the most modern technology available. The facility includes four full-service operating rooms, three minor procedure rooms and a three-tiered graduated recovery area, respecting the individual needs of adult and pediatric patients. The one-story, 19,000-square-foot building is equipped to perform all types of same-day surgical procedures, including arthroscopic, laparoscopic and laser techniques. Every aspect of the center has been designed to provide the ultimate in efficiency and comfort for patients and their families, while offering the highest quality medical care. 6. The Eisenberg Family Center Monmouth delivers nearly 5,000 babies annually - the most in Monmouth and Ocean counties. To accommodate the demand for its services, the hospital recently completed an ambitious expansion of the Eisenberg Family Center to offer additional expansive private postpartum rooms and an additional newborn nursery. The Valerie Fund Children's Center for Cancer and Blood Disorders The center provides comprehensive medical services to children with childhood cancers such as leukemia, lymphomas and neuroblastomas, and blood disorders such as sickle cell anemia and white cell abnormalities. Children and young adults (birth to 21 years of age) with leukemia and other cancers are treated according to the most advanced therapeutic protocols. The center is designated as a "comprehensive treatment center for sickle cell anemia" by the state of New Jersey. CORE FORM, PART III; STATEMENT OF PROGRAM SERVICE ACCOMPLISHMENTS 8. Robotic Surgery Program Monmouth is the first hospital in central and southern New Jersey to introduce robotic-assisted partial knee resurfacing and total hip replacement procedures known as Makoplasty. This surgeon-controlled robotic arm system enables highly accurate alignment and placement of implants for a more natural feel. Additionally, partial knee resurfacing is a minimally invasive option for knee pain sufferer that quickly gets them back to normal activities. This latest example of pioneering robotic surgery at Monmouth Medical Center came on the heels of the hospital's milestone of performing its 2,000th robotic surgery. Monmouth boasts the most experienced robotic surgeons in the state, including fellowship trained physicians in minimally invasive and advanced robotic thoracic surgery, and is the only hospital in the region to employ two surgical robots and the latest technology for minimally invasive complex procedures in adult and pediatric urology, gynecology and thoracic surgery. Monmouth Medical Center's annual 'Roll Out the Ribbons' Campaign Supports the Fight Against All Cancers To raise awareness of cancer in all its forms and educate the public about the importance of early detection and prevention, Monmouth Medical Center created a unique summertime awareness campaign designed to unify the community in the fight against cancer and honor courageous individuals who have and are fighting the battle. Roll Out the Ribbons kicks off on July 4th at Oceanfest on the Long Branch boardwalk, and the colorful ROR tent, offering cancer resources, information, sunscreen, giveaways and an opportunity to fill out a Ribbon of Honor, visits at least 10 public events throughout the summer. The goals are to educate the community about the risk factors associated with cancer and preventive approaches to the disease, and help those affected cope with the challenges ahead. The ROR website is filled with valuable cancer related information, such as links to support organizations, patient stories and local cancer-related events are featured. A focal point of Roll Out the Ribbons is engaging the public in the creation of the Promenade of Hope - a half-mile stretch of railing along the boardwalk in Long Branch delivers messages of hope and healing to the community through its multi-colored ribbons displaying personal messages to symbolize a united front in the fight against cancer. From Memorial Day through Labor Day 2013, MMC proudly hung more than 7,000 ribbons along Long Branch's oceanfront Promenade. Since filling out Ribbons of Honor proved to be so meaningful for so many, the 2014 ROR campaign has now incorporated Expressions of Hope, which encourages participants to reap the benefits of therapeutic writing and journaling by creating and submitting letters to cancer. A public event for cancer patients, caregivers and friends will be held on August 21 and feature therapeutic journaling, meditation, Reiki, and healthy cooking. All but the healthy cooking segment will be led by MMC nurses from the MMC Holistic Nursing Council. Several local schools, including Tinton Falls Middle School, and Shore Regional High School, have become involved in the campaign, with students filling out ribbons and counselors, teachers and students opening a dialogue about cancer in the schools. Local businesses and the community are encouraged to participate in the campaign - one local business held a bake sale for patrons and raised over a thousand dollars and children in a local neighborhood worked together on a lemonade stand to spread the message and raise funds. Funds raised go to patient and family programs of the Leon Hess Cancer Center and the Valerie Fund Children's Center for Cancer and Blood Disorders at Monmouth Medical Center. The Roll Out the Ribbons' "Ribbon of Honor" was designed to represent all types of cancer, as symbolized by its color: lavender, the official color of awareness of all cancers. The colored stripes represent the 12 most common types of cancer: thyroid, prostate, pancreatic, bladder, breast, colorectal, endometrial, kidney, leukemia, lung, melanoma and lymphoma.

Unterberg Children's Hospital Sponsors Beach Safety Week National Rip Current and Beach Safety Awareness Week is observed annually the first week of June. In 2013 and 2014, The Unterberg Children's Hospital at Monmouth Medical Center has sponsored the local "Know BEFORE you go!" campaign that helps educate the community about water safety. The campaign, which is led by the Monmouth County Chapter of the United States Life Guarding Association (including Unterberg Children's Hospital Pediatric Endocrinologist, Mike Barrows, MD), visits local high schools throughout Monmouth County and presents a fun and informative workshop on beach safety and dealing with rip currents. Our sponsorship also enables 128 radio Public Service Announcements throughout June via Press Communications. Importance: Every day about 10 people die from unintentional drowning. Of these, two are children aged 14 or younger. Drowning ranks fifth among the leading causes of unintentional death in the United States. About one in five people who die from drowning are children 14 and younger. For every child who dies from drowning, another receives emergency department care for non-fatal submersible injuries. More than 50% of drowning victims treated in emergency departments require hospitalization or transfer for further care. These non-fatal drowning injuries can cause severe brain damage that may result in long term disabilities such as memory problems, learning disabilities and permanent loss of basic function. 3. Monmouth Medical Center and SCAN Partner on Services for Active Adult Community For more than 25 years, MMC has partnered with the Social Community Activates Network ("SCAN") (formerly the Senior Community Activities Network), located in the Monmouth Mall in Eatontown, in service to the active adult community. Like SCAN, which promotes a culture of lifelong learning, healthy lifestyles and independent living by offering programs that make the community "Healthy, Wealthy & Wise," MMC believes that "Life is Better Healthy" and is dedicated to helping seniors live healthy lifestyles. Throughout the year, Monmouth provides a host of health lectures, screenings and classes led by the hospital's doctors and healthcare professionals. These free programs, which Monmouth advertises monthly in the Asbury Park Press, focus on topics such as heart health, stroke prevention, brain health, cancer awareness, nutrition and stress relief. For example, in 2014, Monmouth teamed with SCAN to offer Take Control of Your Health, a free, six-week workshop to teach strategies for managing daily life when living with chronic conditions such as arthritis, heart and lung conditions, anxiety, diabetes and high blood pressure.
