What is the organization's primary exempt purpose? The purpose of the Coalition shall be to improve the quality of life of all people in Georgia affected by Diabetes through the reduction of health risks related to diabetes.
Describe the organization’s program service accomplishments for each of its three largest program services, as measured by expenses. In a clear and concise manner, describe the services provided, the number of persons benefited, and other relevant information for each program title.
28 Outreach Activity Update GDC is partnering with ADA to help get the word out about resources available around the state. We are in the process of training coordinators to develop local events and conduct diabetes seminars using the ADA Project Power. This will provide a consistency for delivering diabetes information monthly. GDC is also partnering with ADA and Pfizer on June 20th to host a press conference recognizing Diabetic Peripheral Neuropathy Alert Day at the capitol to raise awareness of the dangers of DPN. Rep Cooper, Ramsey, Howard, Watson and Marin are authors of House Resolution 851. The press conference will be held at Atlanta Medical Center at 11am with remote sites in Savannah and Albany Diabetes Centers. GDC board members are welcome and encouraged to participate. More details to come. GDC has been instrumental in working with coordinators in 13 communities and growing. We currently work with coordinators in the following cities: Atlanta, Augusta, Claxton, Columbus, Dalton, Dublin, LaGrange, Millen, Newnan, Savannah, Savannah State University, Sparta and Waycross, GA. As a result of these relationships GDC has become a major player in distributing educational information. Through speaking opportunities, internet, social media sites, and e-newsletter blast GDC, we are continuing to improve our visibility. We continue to list member events and share thru our newsletter and website with members, fellow colleagues, organizations and partners. During May, my travels included work in Millen and Savannah, plus a phone interview with Johanna Sakimura of Everyday Health Magazine and radio interview with Stir It Up-Talk Radio WHCJ Savannah State University hosted by Jacqueline Awe. I also attended a Taking Control of Your Diabetes Conference which provided good information and networking in preparation for our conference in October. Community Leader's Breakfast GDC was successful in hosting a Community Leader's Breakfast in Millen, GA. Over 30 persons attended the breakfast and signed a pledge, representing more than 20 organizations from the Jenkins county community. We are planning another Community Leader's Breakfast in Augusta on June 29th at the Kroc Center from 8am-9:30am. Novo Nordisk is one of the sponsors for this event. We were also successful in hosting a statewide fundraiser in April 2013 with Zaxby's for Diabetes Alert Day raising $1482.00. We are in the planning phase for another Breakfast in Savannah, GA possibly in July. More details to come. Tri-Regional Faith & Community Health Summit/Denim & Diamond Community Awards Reception The Denim & Diamonds Community Awards Reception is our annual recognition and appreciation event hosted by the Georgia Diabetes Coalition (GDC). This year it will also serve as the Welcome Reception for the Tri-Regional Faith & Community Health Summit. The reception type event will recognize national, state and local community members, volunteers, organizations, and sponsors that are collaborating to increase health awareness and implement local programs. To help raise awareness and kick off the Reception and Summit, we are working with Mary Cooper, Novo Nordisk to secure Kendall Simmons, a former NFL player as a celebrity spokesperson and draw for the evening. Special award presentations to members including the Tri-Regional Faith & Community Health Summit Award will close out the evening of networking and entertainment. We are partnering with Region IV of the Department of Health and Human Services to broaden our reach. The 3 day event is scheduled for October 3-5, 2013 and is anticipated to attract more than 1000 participants. Summits will occur in three locations Atlanta, GA, Kansas City, KS and Dallas, TX with the Region IV summit taking place in Atlanta, GA. These events will draw participants from 17 states comprising 45 percent of the total congregations most negatively impacted by the leading health indicators in the United States, as well as countless community-based organizations. Other participating states included in Region IV are Alabama, Florida, Kentucky, Mississippi, North Carolina, South Carolina and Tennessee. These regions suffer from high rates of obesity, infant mortality, diabetes, and cardiovascular disease and are heavily impacted by the social determinants of health. Our goal is to educate, engage, and empower community and faith-based organizations with the tools and resources they need to develop and sustain effective coalitions, health ministries and community health outreach programs. The host site for the Summit is Saint Philip AME Church, Atlanta, GA We need assistance with: -Identifying potential sponsoring companies and contacts from which we can request funding. -Developing award criteria and system for requesting nominations including forms -Reviewing program from last year and making changes for this year -Members to conduct Share Café's at Summit (Diabetes, Eye, Kidney)

29 Expand Active Partnerships/Grow Membership GDC is partnering with AHEC centers and public health districts to promote and publicize their activities and classes through our network by reposting dates to our newsletter and website or forwarding copies of their newsletters to our email date base list. We also work with local support groups in different cities to promote their activities and events via our newsletters and website calendar. This provides consistency in delivering diabetes information monthly. Our membership committee has implemented a membership drive which has resulted in recruiting more members. Through participation in the annual GA Give Day, we have increased donations as well as visibility. The next GA Gives Day is November 13, 2103. Visit our website for more info. Coordinate Statewide Diabetes Activities GDC is a partner with DPCP to help distribute information and resources available around the state. We re-post information about training seminars and workshops as well as Diabetes Self Management classes that are being conducted statewide and online. Through restructuring our website with a resource page, we are able to provide important hyper links to CDC, NIH, ADA, DAA, DPCP and DPH web pages. Awareness Campaign/Marketing The 20/20 Campaign is a digital media campaign conducted by the Georgia Diabetes Coalition and American Diabetes Association to "brand" the 20th of each month as diabetes awareness day. We provide monthly messages and patient education materials to hospitals, employers, churches, pharmacists and diabetes educators around the four most common diabetic complications; blindness, amputations, kidney failure and stroke. Through 20/20 Campaign partners our messages are forwarded to an extended network via email, face book, twitter, newsletters, and church bulletins. Electronic copies of patient education material is also provided for print out and distribution by the recipients as they best see fit. More than 50 partners have signed up to date. Other aspects of the program include working with businesses, organizations and other industry partners to set up educational events on the 20th of each month. This will allow us an opportunity to promote agencies, members and others that are delivering programs to combat these diseases. Partners can promote the 20/20 Campaign and assist in the development of Coalition members throughout the state. The first 20/20 Campaign launched with a media event recognizing Diabetic Peripheral Neuropathy (DPN) Alert Day on June 20, 2013. Rep. Sharon Cooper presented a House Resolution to raise awareness of the dangers of DPN. The media event was held at Atlanta Medical Center. Additional remote sites included Savannah and Albany. Attendance was great at all events and as of today we have enrolled about 40 partners. Special thanks to ADA and Pfizer for assisting with the campaign. By re-branding and optimizing our website, newsletter and 20/20 campaign, we have increased our newsletter visual appeal, sign-ups, face book likes and twitter followings. Become a Recognized Educational Resource GDC has been successful in distributing brochures, information and health related give aways to more than 500+ individuals, organizations and event organizers. We have been instrumental by posting and distributing educational information via CD's, YouTube and tool kits. By partnering with ADA and DAA we anticipate expanding our ability to offer free diabetes education classes to more than 10 communities statewide using the Project Power and Healthy Habits Youth Program. We anticipate reaching more than 1000 persons in the non-metro part of the State. Through our Team Leaders this will deliver free diabetes education monthly. GDC has been instrumental in working with coordinators in 14 communities and growing. We currently work with coordinators in the following cities: Atlanta, Augusta, Baxley, Claxton, Columbus, Dalton, Dublin, LaGrange, Millen, Newnan, Savannah, Savannah State University, Sparta and Waycross, GA. As a result of these relationships GDC has become a major player in distributing educational information. Through speaking opportunities, internet, social media sites, and e-newsletter blast, we are continuing to improve our visibility and anticipate working with over 20 communities before the end of 2014. Strengthen Relationships with CDC and State Diabetes/Monitor Surveillance Data The Georgia Diabetes Coalition celebrated another successful annual "Denim & Diamonds" Community Awards Reception with over 150 guests at the Emory Conference Center Hotel. The evening commenced with an energetic pre-reception networking session and photos with guests arriving in their best pair of Denims and best piece of Diamond or shimmering accessory. Georgia Diabetes Coalition also served as co-host for the Tri-Regional Faith & Community Health Summit with HHS Region IV. The 3 day event was held on October 3-5, 2013 with more than 500 registered participants. This year's Denim & Diamonds Reception also served as the welcome reception for the Tri-Regional Faith & Community Health Summit. Other participating states included in Region IV are Alabama, Florida, Kentucky, Mississippi, North Carolina, South Carolina and Tennessee. These regions suffer from high rates of obesity, infant mortality, diabetes, and cardiovascular disease and are heavily impacted by the social determinants of health. Special thanks to the host location for the Summit, Saint Philip AME Church, Atlanta, GA. Dr. Hogan with the Health Ministry and Rev. McDonald with the Youth Ministry as well as the staff were very accommodating and made us feel welcome. The Denim & Diamonds Community Awards Reception recognized more than forty (40) national, state and local community members, volunteers, organizations, and sponsors that collaborated to increase health awareness and implement local programs. DPCP volunteers and Tri-Regional Faith and Community Health Summit attendees came to the reception to participate in the Tri Regional Community Health Excellence awards given during the awards ceremony. Special Honoree Dr. Guillermo Umpierrez from Grady hospital was supported by dozens of his co-workers who attended and expressed admiration for the doctor. He was recognized for his direct work in the diabetes community. Another CDC honoree were unable to attend due to extenuating circumstances.
