Form 990, Part VI, Section B, line 11	The Form 990 was prepared by an independent CPA firm. It was reviewed by the Treasurer of the Board of Directors and then it was sent to all other members of the Board for review before being filed with the IRS.
Form 990, Part VI, Section B, line 12c	All board members annually sign conflict of interest disclosure statements. Any noted conflicts of interests are reviewed and discussed at monthly board meetings. If a conflict were to arise, the individual would abstain from voting.
Form 990, Part VI, Section B, line 15	All amounts paid to board members are for services provided as firefighters and not for services provided as board members. Board positions are volunteer only. The board of directors approves any compensation amounts. Comparable data is used when approving compensation. The process is documented in the minutes.
