| Part IX-A Line1, Summary of Direct Charitable Activities - The foundation provides mentoring, encour | Part IX-A Line1, Summary of Direct Charitable Activities - The foundation provides mentoring, encouragement, educational opportunities and financial support to exceptional students from lower income families. Students first enter the program in the fourth grade upon referral from their fourth grade teacher. A mentor/student relationship is developed at weekly meetings held at the students school and maintained as long as the student continues to be a high achieving student. Many times these relationships last through college. The foundation also provides financial assistance with books, fees and other costs that would otherwise not be afforded by the student or his/her family. Seven students are still participating. Part XV, Line 2b - Application Format and Required Contents - Referrals will come from fourth grade teachers at selected schools which have high concentrations of underprivileged students. These teachers will complete a questionnaire recommending their 12 best candidates. Part V, Line 2d - Award restrictions or limitations - Awards are restricted to relieve poverty in a poor class of people by aiding needy students/persons. The students are required to maintain minimum grade levels, attend weekly meetings with mentor, exhibit good conduct and to abstain from drug or alcohol use. |
