THE PARKER FAMILY HEALTH CENTER IS A VOLUNTEER-BASED CLINIC FOR THE UNINSURED, SERVING MONMOUTH COUNTY IN CENTRAL NEW JERSEY. OPEN SIX DAYS A WEEK, THE CLINIC LOGS OVER 10,000 PATIENT-VISITS ANNUALLY, WITH OVER 100,000 CUMULATIVE VISITS OVER ITS 14-YEAR HISTORY. HEALTH CARE IS PROVIDED BY 250 MEDICAL AND OTHER VOLUNTEERS WHO DONATE 10,000 HOURS OF SERVICE EACH YEAR TO MAKE HEALTH CARE ACCESSIBLE TO THOSE IN NEED. THE CLINIC PROVIDES COMPREHENSIVE PRIMARY HEALTH SERVICES FREE OF CHARGE TO THE UNINSURED, WITH A FOCUS ON PREVENTIVE CARE, CHRONIC DISEASE MANAGEMENT AND HEALTH EDUCATION. SERVICES INCLUDE WOMEN'S HEALTH, PEDIATRICS, DIABETES AND OTHER CHRONIC DISEASE MANAGEMENT, CANCER SCREENING, VISION CARE, EMPLOYMENT AND SCHOOL PHYSICALS, ADULT AND CHILD IMMUNIZATIONS, AND SOCIAL SERVICE AND LEGAL REFERRALS.

KEY SERVICES INCLUDE THE FOLLOWING: CHRONIC DISEASE MANAGEMENT: MANY CLINIC PATIENTS SUFFER FROM CHRONIC DISEASES, INCLUDING OVER 300 DIABETICS ENROLLED IN A COMPRENSIVE DISEASE MANAGEMENT PROGRAM. IN 2013, 64 PERCENT OF DIABETIC PATIENTS AT PARKER ACHIEVED GOOD CONTROL OF BLOOD SUGAR, COMPARED WITH 47 PERCENT IN NATIONAL DATA FOR SIMILAR PATIENTS. IN 2013, THE CLINIC BEGAN AN ASTHMA INITIATIVE AIMED AT ALIGNING TREATMENT AT THE CLINIC WITH ESTABLISHED GUIDELINES TO BETTER SERVE OVER 100 PATIENTS WITH ASTHMA. PREVENTIVE CANCER SCREENING: IN 2013, THE PARKER CENTER LAUNCHED A NEW INITIATIVE THAT IS MAKING GENETIC TESTING FOR THE BRCA GENES THAT GREATLY INCREASE THE LIKELIHOOD OF BREAST AND OTHER CANCERS AVAILABLE TO WOMEN PATIENTS. IN ADDITION, OVER THE YEAR, 271 WOMEN PATIENTS RECEIVED MAMMOGRAMS AND 100 PATIENTS WERE SCREENED FOR CERVICAL OR COLON CANCER. CHILDREN'S HEALTH: THE PARKER PEDIATRIC PROGRAM FOCUSES ON GIVING VULNERABLE CHILDREN A HEALTHY START IN LIFE. IN 2013, 164 CHILDREN RECEIVED COMPRENSIVE CARE, INCLUDING WELL-CHILD PHYSICALS, IMMUNIZATIONS AND ORAL HEALTH CARE. THE CLINIC'S IMMUNIZATION RATE FOR CHILDREN EXCEEDS BOTH STATE AND NATIONAL AVERAGES. OTHER SERVICES: THE CLINIC ENSURES ACCESS TO ESSENTIAL DRUG-BASED TREATMENT BY ASSISTING PATIENTS WITH APPLICATIONS TO PHARMACY ASSISTANCE PROGRAMS. PARKER HOSTS THIRD-YEAR UMDNJ MEDICAL STUDENTS WHO SHADOW PARKER VOLUNTEER PHYSICIANS, GAINING VALUABLE CLINICAL EXPERIENCE AND AN INTRODUCTION TO COMMUNITY SERVICE. TO HELP ADDRESS THE SOCIAL FACTORS THAT IMPACT HEALTH FOR THE LOW-INCOME PATIENT POPULATION, A CLINIC SOCIAL WORKER PROVIDES REFERRALS TO LEGAL ASSISTANCE AND SOCIAL SERVICES. THE SOCIAL WORKER IS ALSO HELPING ENROLL ELIGIBLE PATIENTS IN MEDICAID AND OTHER GOVERNMENT HEALTH INSURANCE PROGRAMS, AS WELL AS PROVIDING GUIDANCE TO ASSIST PATIENTS IN APPLYING FOR SUBSIDIZED COVERAGE ON THE HEALTH INSURANCE EXCHANGE.
