Form 990, Part III, Line 4d: Other Program Services Description
OTHER PROGRAM SERVICES 4: Behavior Modification: This program is specially designed for consumers with higher level behavioral challenges. The purpose of this program is to increase consumers' level of independence, and decrease their level of behaviors, while they are involved in work and other meaningful activities. We "target" behaviors on a daily basis so that each individual can learn how to self-manage and cope with a variety of situations and environments. The program requires a 1:3 ratio in order to ensure the safety of each participant, and to provide adequate training and assistance. We also have two Behavior Consultants who help our staff increase their understanding of the function of the behavior and assist consumers with communicating their needs effectively. Once a consumer is able to successfully meet the goals identified in his or her Behavior/Individual Service Plan, they will be encouraged to transition to one of our less restrictive programs. (Approx. 27 adults) OTHER PROGRAM SERVICES 5: Work Activity Program: This program focuses on small-group and individual employment that helps the adult meet their career goals and receive a steady paycheck. Participants enter the program by joining paid group work sites where they can learn and hone skills. With additional coaching, selected adults are supported during a one-year to one-and-a-half year timeframe to move on to individual job placements in the community. The result is increased independence and self-reliance. (Approx. 8 adults) OTHER PROGRAM SERVICES 6: Senior Program: This program is open to those who have reached a minimum age of 50 years. The primary focus is the promotion of an environment that is conducive to the discovery of recreational alternatives, the improvement of an individual's leisure skills, and enhancement of the retirement experience for our senior population. Recreational alternatives and community integration is emphasized through self-determined choices and specifically-tailored experiences aimed at the senior-age individual. Seniors who are part of the program are offered the option of continuing to earn a paycheck. The staff-to-senior ratio is 1:6, thereby providing intimate and effective programming. Our Senior Program participants are encouraged to become involved in our many site-based programs, such as art, horticulture, computers and swimming. Our programming for seniors is specifically-tailored to meet the needs of these older adults. (Approx. 68 adults)
