Form 990, Part VI, Section A, line 3	The Trust has contracted with a third-party administrator (Benefit Solutions, Inc.) to provide bookkeeping and administrative services.
Form 990, Part VI, Section A, line 7a	An individual who satisfies the qualification of a Trustee may be nominated as a candidate for Trustee by a majority of the Trustees, excluding Trustees who are in the final year of their term; or by written petition submitted to the Board of Trustees signed by the lesser of ten participating employers or at least 10% of all participating employers. A Trustee in the final year of their term, and satisfying the qualifications of a Trustee, may be nominated for a successive term. Individual candidates nominated for Trustee shall be elected by vote of the participating employers, who will each have only one vote. If no candidate receives a majority of the votes cast by participating employers, a runoff election between the two candidates who received the most votes shall be held until a single candidate receives a majority of votes cast by participating employers. The election for Trustees shall be conducted by mail ballot at the expense of the Trust. Each Trustee who is elected shall consent to accept their election as Trustee in writing.

Form 990, Part VII, Section A, Line 1A:	The Trust does not compensate any of the members of the Board of Trustees. The Board is comprised of four individuals, all of whom serve on a volunteer basis. In their roles as Trustees, all are fiduciaries and understand their obligations under the Trust Agreement and the Trust's policies and procedures. The Trust does not readily have access to information related to compensation paid by other entities to the Trustees. Additionally, the amount of time spent on activities related to being a Trustee is less than 400 hours per year per Trustee.
