For all instances reported, PSI performed investigations to determine the extent of the issue, reported the value to any funders whose activities were impacted by the diversions, ensured that funds were returned from other sources, and identified improvements to internal controls to reduce the risk of similar occurrences. The staff involved in these instances are no longer employed by PSI. In a separate incident, PSI suffered a loss of product with a value of $453,316 due to a theft of injectables from its country's central warehouse after the local Ministry of Health warehouse encountered a fire which resulted in a loss of all other injectables within the country. No PSI staff have been implicated in this incident and it appears to have been perpetrated by third parties.
