SIL PROVIDES WWORKSHOPS & INDIVIDUAL TRAINING TO CONSUMERS TO INCREASE THEIR EFFECTIVENESS AS SELF-ADVOCATES. SIL ALSO CONDUCTS WORKSHOPS FOR PROFESSIONALS ON ISSUES IMPACTING PERSONS WITH DISABILITIES AND THEIR ABILITY TO PARTICIPATE IN COMMUNITY LIFE. PERSONAL CARE ASSISTANCE PROGRAM- SIL IS A CONTRACTOR FOR THE STATE OF OHIO'S PERSONAL CARE ASSISTANCE PROGRAM. THIS PROGRAM PROVIDES SUPPORTS TO INDIVIDUALS WITH SEVERE PHYSICAL DISABILITIES IN THEIR APPLICATION FOR PERSONAL CARE ASSISTANCE FUNDS. ONCE ON THE PROGRAM, STAFF PROVIDE TECHNICAL SUPPORTS TO PARTICIPANTS REGARDING PERSONAL ASSISTANCE MANAGEMENT (E.G. HIRING, FIRING, DAY-TO-DAY PAS MANAGEMENT AND DOCUMENTATION). INDEPENDENT LIVING SKILLS TRAINING (ILST) - SIL PARTNERS WITH A LOCAL DISABILITY RELATED ORGANIZATION TO PROVIDE ON-SITE ILST TO CONSUMERS. THIS HAS BEEN A HUGE SUCCESS AND MAKES A DIFFERENCE IN THE LIVES OF CONSUMERS SERVED. INDIVIDUALS ARE ALSO OFFERED THE OPPORTUNITY TO PARTICIPATE CUSTOMIZED; INDEPENDENT LIVING SKILLS TRAINING. ILST CAN INCLUDE INSTRUCTION IN TOPICS SUCH AS BUDGETING, COOKING, BALANCING A CHECKBOOK, LAUNDRY ETC PEER SUPPORT PROGRAM -- PEER SUPPORT GETS TO THE HEART OF SIL'S SERVICE DELIVERY SYSTEM AND CAN MAKE THE DIFFERENCE ISOLATION OR AN INDIVIDUAL'S ABILITY TO PARTICIPATE IN COMMUNITY LIFE. PEER MENTORS PROVIDE CONSUMERS WITH POSITIVE ROLE MODELS, PROBLEM SOLVING AND COPING SKILLS. SUPPORT GROUPS ARE ALSO AVAILABLE AS WELL. EQUIPMENT LOAN PROGRAM - SIL'S EQUIPMENT LOAN PROGRAM IS OPERATED IN A FIRST-COME, FIRST SERVED BASIS. TYPES OF EQUIPMENT THAT HAS BEEN LOANED OUT INCLUDES: WALKERS, WHEELCHAIRS, CANES ETC. NURSING HOME TRANSITION PROGRAM- SIL IS ONE OF THE DESIGNATED PROVIDERS OF TRANSITION SERVICES UNDER HOME CHOICE, OHIO'S MONEY FOLLOWS THE PERSON (MFP) DEMONSTRATION PROGRAM. THE PROGRAM PROVIDES STAFF AND LIMITED FINANCIAL SUPPORTS TO INDIVIDUALS INTERESTED IN RELOCATING TO THE COMMUNITY AFTER AN EXTENDED FACILITY STAY. WORKING CLOSELY WITH CONSUMERS, STAFF IDENTIFY AND ADDRESS SUPPORTS NEEDED TO BE SUCCESSFUL IN THE COMMUNITY. SIL STAFF RELOCATED FIFTY (50) INDIVIDUALS DURING THIS FISCAL YEAR.
