During the current fiscal year, the Crisis Intervention and Recovery Center, Inc. had volunteers give 39.25 hours of service to the organization's exempt purpose and its programs, which is valued at $393. Additionally, the board members donate their time and service to the organization.

The following policy is in place: documents in possession of the Crisis Intervention and Recovery Center, Inc., that are deemed public record may be shown to any person/persons under the following:A. Records may be examined between the hours of 8:30am and 5:00 pm on scheduled work days, Monday through Friday.B. A staff member must be present to assist in facilitation of examination.C. In the case where client information is contained in the records to be examined, the agency will screen out any document or information that would, in any way, disclose the identity of a person receiving services from the agency.D. The person/persons wishing to examine multiple documents will be required to submit a letter stating the nature of the records requested, year of records requested, and the preferred time, in relation to item 1A.E. The Executive Director shall inform the person/persons/requesting documents when the documents will be made available. All requests will be attempted to be fulfilled within 48 hours, where reasonable.F. Any person/persons requesting photocopies of any documents will be charged according to the agency policy on "Personal use of copier and fax machine."G. documents will not be allowed to be taken off the premises.

The Board of Trustees conducts the annual evaluation of the executive director. At that time, the compensation is discussed and adjusted based upon the evaluation and what can be supported by the agency budget. The agency uses a salary survey prepared by the Ohio council of behavioral healthcare providers for comparative data. All other compensation is then presented to the board of trustees for approval. The board meeting minutes would serve as documentation of the review.

Copies of the Form 990 are distributed to the Board of Trustees at the board meeting prior to the due date of the return. The Form 990 is presented at the meeting by the Finance Director. The form is then signed by an authorized officer for filing.

OTHER PROGRAM SERVICES 4: Hotline: Thousands of crisis hotline calls were answered in fiscal year 2013. OTHER PROGRAM SERVICES 5: Detoxification - Sub acute detoxification services for those addicted to alochol and other drugs were provided to 723 clients in our detoxification unit during fiscal year 2013. clients stayed an average of 3.5 days. OTHER PROGRAM SERVICES 6: Community education and prevention programs involve various speaking engagements on numerous topics, consultation services, and suicide prevention activities for groups and individuals. OTHER PROGRAM SERVICES 7: Miscellaneous Programs such as the free space women's program provided services to help women live violence free and substance free lives. There is also a home program to provide services to prevent and reduce homelessness.
