United Way's 10-year goal for health is to connect over 200,000 people with available, preventative health services. We focus on the underling factors that lead to early death so we can increase the number of households living longer, healthier lives. We work to stabilize families and individuals in immediate crisis and mobilize our community to create safe places and a healthy environment. United Way also focuses on funding programs in United Way partner communities that best increase physical activity, healthy eating behaviors and work towards eliminating desserts. By reducing barriers to health care access, we connect people to consistent primary, mental and social health services. THROUGH THIS INITIATIVE, 74,220 INDIVIDUALS CONNECTED WITH A PRIMARY CARE PHYSICIAN, AN AVERAGE OF 37,000/YEAR. TO DATE, 500,000 PEOPLE EXPERIENCED REDUCED BARRIERS TO CARE (FINANCIAL, LANGUAGE, LOCATION, AND KNOWING A PROVIDER).

United Way's 10 year goal for education is to help 50,000 underperforming middle school kids enter high school ready to succeed. We will accomplish this goal by laser-focusing on two key transition periods in a child's life: early childhood education and middle school transition into high school. We ensure children enter school ready to learn by expanding access to and participating in high-quality preschools and home visitation programs and ensuring parental involvement through outreach and education programs. We also prepare middle school kids to enter high school ready to succeed by increasing resources for existing community schools in United Way partner communities and bringing community leadership and planning into schools through local advisory boards. THROUGH THIS INITIATIVE, 7,500 MIDDLE SCHOOL STUDENTS ARE READY FOR HIGH SCHOOL SUCCESS. TO DATE, 92% OF THESE STUDENTS WERE PROMOTED TO THE NEXT GRADE LEVEL; 95% OF FAMILIES OF PRESCHOOL CHILDREN AND 84% OF FAMILIES OF MIDDLE-SCHOOL CHILDREN REPORTED FEELING MORE CONFIDENT TO SUPPORT THEIR CHILDREN'S LEARNING AT HOME; AND 10,177 KIDS WERE READY FOR KINDERGARTEN.

Intended Uses of Endowment Funds Schedule D, Part V, Line 4 The intended uses of the United Way of Metropolitan Chicago endowment funds include funding innovative programs that address critical and emerging needs, ensuring that current needs are met, and providing resources to respond quickly and effectively to unexpected crisis.
Uncertain Tax Positions (FIN 48) Schedule D, Part X, Line 2 UWMC, South-Southwest, Northwest, North Shore and DuPage/West Cook have received favorable determination letters from the Internal Revenue Service, stating that they are exempt from Federal income taxes under the provisions of 501(c)(3) of the Internal Revenue Code of 1986, except for income taxes pertaining to unrelated business income. Accounting guidance requires tax effects from uncertain tax positions to be recognized in the financial statements only if the position is more likely than not to be sustained if the position was to be challenged by a taxing authority. Management has determined that there are no material uncertain positions that require recognition in the financial statements. Additionally, no provision for income taxes is reflected in these financial statements. There is no interest or penalties recognized in the financial statements. The tax years ending 2009, 2010, 2011 and 2012 are still open to audit for both Federal and state purposes.

("uwmc") is an illinois non-profit philanthropic corporation whose mission is to improve the lives in the metropolitan chicago area by mobilizing caring people to invest in the community where their resources are needed the most. as of june 30, 2013, uwmc had four member united ways ("muws"): south-southwest suburban united way northwest suburban united way north shore untied way united way of dupage/west cook effective july 1, 2013, northwest suburban united way and north shore united way merged to form united way north-northwest. uwmc and the muws serve as key entities in the metropolitan chicago region generating, convening and coordinating resources across donors, service providers, government and civic leaders to deliver large-scale, measurable and sustainable impact across the region. the vision of uwmc and the muws is that metropolitan chicago is a region of thriving communities where all individuals and families are able to achieve their fullest potential. uwmc and the muws implement the live united 2020 community-impact plan to change the story for individuals and families in 41 suburban and 17 city communities of greatest need through support and interventions in the areas of education, income, health and basic needs, the building blocks of self-sufficiency and quality of life. the target funding strategy approved by the board of directors is 40% for education and 20% each for income, health and basic needs. as part of the live united 2020 plan, uwmc and the muws are launching live united neighborhood networks. these networks take community change and impact to the next level. working with a lead partner in the community, uwmc and the muws organize and integrate a network of resources in education, income and health in a central location, usually a school, in one of our 58 communities of greatest need. this model allows families to access the necessary mix of quality programs under one roof. united way brings together agency services, financial support, expertise, volunteers and corporate partners to improve the educational achievement, financial stability, and health of people in need. uwmc's first live united neighborhood network is in brighton park on chicago's southwest side. in partnership with the brighton park neighborhood council, which operates within three middle schools, uwmc is providing afterschool programming, health services, english as a second language instruction and more. the muws will be launching their first live united neighborhood network in 2014 in west chicago. the goal for uwmc and the muws is to launch 10 networks in chicago and 5 in the suburbs by 2020. in addition to the live united neighborhood networks, uwmc is implementing an initiative for donors to champion a school. a donor's gift supports programming at a middle school in an underserved community to put more children on the path toward high school graduation. through the donor investment, the donor partners with uwmc and community partners to offer services and programs that meet the unique needs of children and families within a particular middle school. these services include (but are not limited to) increasing out-of-school time learning, placement of parent mentors inside the classroom, technology and curriculum enhancements, and sports/fitness facility upgrades. preparing our youth for graduation, post-secondary education and career trajectory is critical to ensuring that they and their families are financially secure and that the region's economy is competitive nationally and globally. through the champion a school initiative, uwmc is zeroing in on the need to bolster early middle school programming in communities of greatest need in order to put more children on the path toward high school graduation.

UWMC ACTS AS AGENT FOR THE MUWS IN PROCESSING PLEDGES, RECEIVING CASH ON PLEDGES, AND PAYING MUW ALLOCATIONS AND EXPENSES. Functional Expenses Form 990, Part IX, Summary UWMC CALCULATES ITS 2012 TAX YEAR (FISCAL YEAR ENDED JUNE 30, 2013) OVERHEAD RATE USING THE SUM OF MANAGEMENT AND GENERAL TOTAL EXPENSES (5,180,199; COLUMN C) AND FUNDRAISING TOTAL EXPENSES (4,619,431; COLUMN D) REPORTED IN PART IX (STATEMENT OF FUNCTIONAL EXPENSES) DIVIDED BY THE TOTAL REVENUE REPORTED IN PART VIII, LINE 12 OF COLUMN A (44,854,684). THIS OVERHEAD RATE FOR THE 2012 TAX YEAR IS 21.8% COMPARED TO 18.7% FOR THE 2011 TAX YEAR (FISCAL YEAR ENDED JUNE 30, 2012). A SUPPORT SERVICE FEE IS ASSESSED TO EACH MUW, HOWEVER, NOT ALL COSTS ARE PASSED ON TO THE MUWS. THEREFORE, THE UWMC OVERHEAD RATE WILL BE IMPACTED BY THESE COSTS.
