FORM 990, PART III, LINE 4A The mission of the Muscular Dystrophy Association (MDA) is to save and improve lives for people fighting muscle diseases. In addition to funding research, MDA does this by supporting and empowering families through its services programs. MDA provides services to those affected by any of the disorders in its program, irrespective of age, race, creed, or gender. MDA's program is designed to assist those affected by muscular dystrophy and related diseases of the neuromuscular system, which affect children and adults. A complete list of diseases covered can be obtained by written request to MDA at 3300 East Sunrise Drive, Tucson, Arizona 85718 or via www.mda.org. MDA maintains the most comprehensive services program of any voluntary health agency in the country. This aspect of MDA's program accounted for over $65,014,202 of its 2013 expenditures. MDA's program - ranging from a nationwide network of clinics to assistance with essential support services - is administered through its field offices across the United States and in Puerto Rico. Caring MDA staff in these offices provides support for local families upon diagnosis and throughout their neuromuscular disease journey. MDA Clinics: MDA maintains a network of 200 hospital-affiliated clinics located at medical institutions and university-based facilities. Each year MDA provides tens of thousands of medical visits through its clinic program. With a referral from a physician who suspects a neuromuscular disorder, individuals are eligible for a diagnostic evaluation at an MDA clinic. Once confirmed with a diagnosis covered by MDA, individuals can receive follow-up care from clinic team members using a multidisciplinary team approach. Annual physical, occupational, respiratory and speech therapy consultations, as well as annual flu inoculations, are among the many services available through MDA clinics. MDA clinics also are essential to the advancement of neuromuscular disease research and serve as focal points for the clinical application of scientific advances designed to treat these disorders, including clinical trials. In 2012 the Association established the MDA U.S. Neuromuscular Disease Registry, with the aim of improving survival and quality of life for those with neuromuscular diseases, as well as expediting clinical trials. The registry was piloted at 25 MDA clinics in 2013, with plans in place to expand the registry to MDA's entire clinic network. As part of its clinic program, the Association has over 40 amyotrophic lateral sclerosis (ALS) centers across the country (listing available at mda.org). Equipment Assistance: Through its Equipment Inventory Program, MDA provided thousands of adaptive devices in 2013 - at no cost to those served by the Association. Examples of equipment provided include walkers, wheelchairs, communication technologies and hospital beds. MDA also assisted with repairs to all types of durable medical equipment for those it serves. Summer Camp: In 2013, thousands of children with neuromuscular disease enjoyed a week of fun and friendship at one of nearly 80 MDA summer camps across the country. Support and Information Services: Support and information were provided through MDA's nationwide network of over 150 local support groups and educational seminars. Support also was available through MDA's care coordination tool, www.mda.org/myMuscleTeam, which enables families to seek assistance from friends and loved ones by posting household tasks and caregiving needs for which help is being sought, such as transportation to medical appointments or meal preparation. MDA staff also assisted individuals by advising them about federal, state and community services for which they may be eligible. MDA's transitional services offered support and resources for youth with neuromuscular disease who are entering adulthood, including an online Transitions Center at www.transitions.mda.org/. FORM 990, PART III, LINE 4B The Muscular Dystrophy Association, Inc., (MDA) is a not-for-profit organization incorporated on June 6, 1950, under the laws of the State of New York. MDA supports worldwide research to find the causes of and develop therapies and cures for muscular dystrophies and related neuromuscular disorders. MDA's programs are supported almost entirely by individual private contributions from the general public. MDA neither receives fees from those it serves nor applies any means test for services. The diseases supported by MDA are the muscular dystrophies (among which are Duchenne and Becker); motor neuron diseases, including amyotrophic lateral sclerosis (ALS, Lou Gehrig's disease) and spinal muscular atrophy; the peripheral nerve disorders Charcot-Marie-Tooth disease and Friedreich ataxia; inflammatory myopathies; disorders of the neuromuscular junction; metabolic diseases of muscle and other myopathies. Most of the disorders in MDA's program have genetic underpinnings. Most cause progressive weakness resulting in significant mobility impairment. Some of the more than 40 neuromuscular diseases covered by MDA are life-shortening. As reported by the U.S. Department of Health and Human Services, National Institutes of Health (NIH), in the United States alone, muscular dystrophy and other neuromuscular disorders are estimated to affect some 1 million people. MDA supports hundreds of research teams worldwide, primarily at academic institutions, through its investigator initiated research grants program. The basic science projects funded by MDA, such as disease gene discovery, gene transfer technology, and cellular regulation studies, are now being applied to the development of therapies. The Association is currently funding therapeutic strategies such as gene- and stem cell-based therapies, biologics, small molecules, antisense oligonucleotides, RNA interference, epigenetic regulation, permanent gene repair and more. MDA is currently sponsoring research grants in the United States and a dozen foreign countries. The Association's research program accounted for some $29,354,830 of its expenditures for the 2013 calendar year. MDA's Scientific and Medical Advisory Committees, whose members are among the nation's foremost scientists and physicians in the field of neuromuscular disease, carefully evaluate all Research Grant and Development Grant proposals submitted to the Association. MDA maintains a diverse program of basic research, which advances investigations of possible treatments for neuromuscular diseases, muscle function, regulation and regeneration; biochemical changes involved in muscle disease; the genetics of neuromuscular disease; and the interaction of nerve and muscle. This work is crucial for providing a sound scientific foundation upon which practical advances against disease can be built. Additionally, the Association's translational research program is focused on milestone driven contracts with the biotech industry, pharmaceutical companies and academic investigators for research that is directly relevant to bringing new therapies to market. Advisors to MDA's translational research program include a number of top neuromuscular disease researchers, as well as representatives of NIH and industry. Recent Research Accomplishments Accelerating therapy development and reducing barriers to move therapies into human clinical trials is a focus of MDA's research program. Through its basic and translational research grants programs MDA is supporting preclinical development and early phase human testing of strategies and compounds designed to slow progression of neuromuscular diseases and important strides are being made as a result of MDA-support research. Exon skipping is a therapeutic strategy that coaxes muscle cells to ignore, or "skip over," a flawed section of genetic instructions for the dystrophin protein, which is absent in people with DMD. MDA funded much of the early work that led to exon skipping drugs that are now showing encouraging results in human clinical trials, and continues to develop the strategy for DMD. A 48-week phase 2b clinical trial of the exon-51 skipping drug, eteplirsen, reported that the drug was safe and well tolerated, resulted in significant production of dystrophin in the region of muscle fibers where it is required, and slowed the decline in walking ability in study participants. In an extension of that study reported in 2013, the drug remains safe and well tolerated and walking ability remained stable at 96 weeks. The synthetic molecules that promote exon-skipping are termed antisense-oligonucleotides (AONs) which are custom designed to target specific regions of the DNA or RNA genetic material. In addition to exon skipping to repair faulty dystrophin genes, MDA-supported researchers are employing AONs to develop innovative therapeutic approaches for a number of other diseases, including SMA, myotonic dystrophy, ALS, and FSHD. In 2013, the experimental antisense drug ISIS-SMNRx was shown to be safe and well tolerated in early phase testing in infants and children with spinal muscular atrophy and a phase 2 study of this drug was launched in infants. ISIS-SMNRx, developed by Isis Pharmaceuticals in Carlsbad, Calif.; MDA supported the laboratory development of this drug through a grant to Adrian Krainer at Cold Spring Harbor (N.Y.) Laboratory. For myotonic dystrophy type 1, scientists are using AONs to block disease-causing genetic instructions in cells and animal disease models. Planning for human clinical trials of this therapeutic strategy for myotonic muscular dystrophy is underway. With support from MDA Dr. Charles Thornton at the University of Rochester has developed a Myotonic Dystrophy Clinical Research Network to standardize methods for evaluating those with myotonic dystrophy types 1 and 2, in preparation for clinical trials in these disorders. MDA also supports a Clinical Research Network focused on DMD and another on ALS. In 2013, the DMD network announced results of its study of infants and toddlers with DMD, demonstrating that signs of the disorder are apparent at these very young ages. This information will help inform design of clinical trials to include this population in the future. Additionally in 2013 awarded the DMD Clinical Research Network a grant to test prednisone, the only drug currently available for the treatment of DMD, in boys 30 months of age and younger. Work of the ALS clinical research network is focused on quality measures for ALS, in conjunction with the American Academy of Neurology, as well as the effect of diet in the progression of the disorder. In ALS, MDA continued to fund studies aimed at better understanding the causes of ALS and identifying targets for therapy development. An important finding reported in 2013 was that generation of toxic RNA plays a role in C9ORF72 ALS and frontotemporal dementia, the most common genetic cause of ALS. These findings provide the basis for further investigation of antisense-based therapies as a treatment for this form of ALS. Another gene that is involved in causing ALS is the FUS gene. MDA-supported researchers linked ALS-causing mutations in the FUS gene to an impaired ability to repair DNA, and that a failure to repair DNA damage appears to play an important role in ALS. MDA also funded a ALS exome sequencing project to help identify new genes associated with ALS. In August 2013, MDA funded its second grantee under the Bridge-to-Industry (B2I) program, a pilot project designed to train promising researchers in translational research by providing experience both in academia and the biopharmaceutical industry. Translational research refers to the lengthy and complex "translation" of promising laboratory findings into marketable drugs. Dr. Christopher Penton will study compounds that boost muscle repair and dampen muscle scar formation at the University of Arizona and pharmaceutical company Sanofi. To promote timely sharing of results and to encourage collaborations among investigators MDA hosts a biennial Scientific Conference. With a focus on turning research into treatments as quickly as possible, more than 500 participants from the academic, corporate drug development and clinical arenas gathered in Washington, D.C. in April 2013 for this international meeting. More than 70 oral presentations and nearly 200 scientific posters were presented at the event, covering a wide range of diseases under MDA's umbrella MDA remains committed to supporting cutting-edge research by top scientists and clinicians throughout the world-research. The focus of MDA's research effort in the future remains the advancement of strategies to apply the technologies resulting from years of basic research toward the development of effective therapies. FORM 990, PART III, LINE 4C In the year ended December 31, 2013, the Muscular Dystrophy Association (MDA) expended $18,075,948 on its professional and public health education program. MDA annually provides the medical profession, scientific community, general public and people affected by muscle diseases with timely and thorough information about MDA's programs and the 43 different diseases they cover. MDA successfully worked with media outlets nationwide in 2013, placing thousands of documented news stories about its mission to save and improve lives for people fighting muscle disease with network, syndicated and local broadcast outlets, print news publications, and online, including social media mentions. Representing millions of impressions collectively, the valuable earned media placements were reinforced by thousands of no-cost public service advertisements. In addition to free banner advertisements on many high-traffic websites, a substantial number of MDA's "Progress that Moves Muscles" PSAs were published by newspapers and other periodicals; and broadcast by national, regional and local television, cable and radio stations. MDA publications and extensive online postings produced a steady stream of information about MDA-funded research and health care services, as well as insightful articles on relevant legislation and inspiring individuals. In addition to posting two to three online news articles per week during 2013, MDA produced four print issues of its award-winning national magazine Quest. In addition, MDA produced and distributed a variety of educational videos for families affected by muscular dystrophy and related diseases. These videos were reinforced by an extensive array of program-specific spots broadcast during the 2013 MDA Show of Strength Telethon viewed by tens of millions worldwide on the ABC network. Among these spots were special profiles of people living with muscle diseases; research updates; educational spots on the myriad medical and health care services available through MDA's national network of 200 medical clinics; a spotlight on MDA's extraordinary summer camp program for children ages 6 to 17; and much more. MDA also continued to enhance its popular website at mda.org, as well as many other program-specific sites. Internationally recognized as a key source of information about neuromuscular diseases, total 2013 page views at mda.org surpassed 5.7 million, with unique visitors exceeding 1.7 million for the year. MDA also facilitated and promoted a variety of webinars and conference live streams to give the families it serves, as well as medical and health care professionals, ample opportunities to ask questions about diseases, research, services and many other topics.
