Schedule R, Part II	The Lutheran Church-Missouri Synod is the sponsoring organization of the Concordia Health Plan VEBA. At 12/31/13 there were 5,500 affiliated churches, schools, and related entities enrolled in the plan of which 3,373 made contributions for at least one employee to be enrolled in the plan.
Schedule R, Part V, Line 1d	Concordia Health Plan (CHP) maintains a line of credit with Concordia Plan Services (CPS), a 501(c)(3) organization. CHP has delegated administration of CHP to CPS. CHP compensated CPS for the administration function through a management fee. The rate of interest was 0.5% and the balance at 12/31/13 was $674,945.
Schedule R, Part V, Line 1n	The Lutheran Church-Missouri Synod (LCMS) is the sponsoring organization of the VEBA. Concordia Plan Services administers the benefit plans/trusts of the LCMS. Concordia Health Plan is one of the plans/trusts administered by CPS. All the plans/trusts operate within the same building and share resources.
