CORE FORM, PART III Background ========== Community Medical Center ("CMC") is a general medical and surgical hospital. CMC is recognized by the IRS as an internal revenue code section 501(c)(3) tax-exempt organization. Pursuant to its charitable purposes, CMC provides medically necessary healthcare services to all individuals in a non-discriminatory manner regardless of race, color, creed, sex, national origin, religion or ability to pay. Moreover, CMC operates consistently with the following criteria outlined in IRS revenue ruling 69-545: 1. CMC provides medically necessary healthcare services to all individuals regardless of ability to pay, including charity care, self-pay, Medicare and Medicaid patients; 2. CMC operates an active emergency department for all persons which is open 24 hours a day, 7 days a week, 365 days per year; 3. CMC maintains an open medical staff, with privileges available to all qualified physicians; 4. Control of CMC rests with its board of trustees and the board of trustees of Barnabas Health, Inc. Both boards are comprised of independent civic leaders and other prominent members of the community; and 5. Surplus funds are used to improve the quality of patient care, expand and renovate facilities and advance medical care, programs and activities. The operations of CMC, as shown through the factors outlined above and other information contained herein, clearly demonstrate the hospital provides substantial community benefit and that the use and control of CMC is for the benefit of the public and that no part of the income or net earnings of the organization inures to the benefit of any private individual nor is any private interest being served other than incidentally. CMC's sole corporate member is Barnabas Health, Inc. ("BH"), a tax-exempt New Jersey non-profit parent corporation. Barnabas Health is a tax-exempt integrated healthcare delivery system consisting of a group of affiliated healthcare organizations. This integrated network of healthcare providers throughout the state of New Jersey provides substantial community benefit. The sole member or stockholder of each entity is either BH or another BH affiliate controlled or owned by BH. CMC, like its corporate parent is a not-for-profit organization. BH is the largest multi-hospital system in New Jersey and provides substantial community benefit. BH consists of free-standing acute care hospitals, a free-standing psychiatric hospital, Medicare-certified home healthcare and hospice programs, multi-specialty ambulatory care facilities, primary care centers, outpatient behavioral health centers and other entities. BH provides medically necessary healthcare services to all individuals regardless of ability to pay. Moreover, BH provides healthcare services to patients who meet certain criteria defined by the New Jersey Department of Health and Senior services without charge or at amounts less than established rates. Hospital Statistical Information ================================ CMC is a non-profit general medical and surgical hospital located in Toms River, Ocean County, New Jersey. CMC is comprised of 587 adult beds, a 32 bassinet nursery and 25 transitional care (skilled nursing) beds. During 2013, CMC serviced nearly 25,000 inpatients and over 84,800 emergency department visits. The emergency department is open to all individuals 7 days a week, 24 hours per day, 365 days per year. Mission Statement ================= Community Medical Center, a not-for-profit acute care hospital serving the Ocean County region, is committed to enhancing the overall health status of the community by providing the highest quality healthcare and related services, including continuing medical education for our medical staff. Accomplishments/Milestones/Recognitions/Awards ============================================== CMC has been recognized with distinguished awards for clinical excellence. Some of these are listed below: - The hospital was surveyed by the Joint Commission and received its triennial re-accreditation for the hospital, home care and hospice programs. - The hospital is the recipient of the Joint Commission - gold seal of approval for acute coronary syndrome, heart failure, cardiac rehabilitation, total joint replacement - hip & knee, and stroke care. - The New Jersey Department of Health designated CMC as a primary stroke center. - The Commission on Cancer of the American College of Surgeons designated CMC as a community hospital comprehensive cancer program. This year, the staff surpassed the 125,000-pound benchmark, collecting an eight-year cumulative total of 125,289 pounds of food, 18,243 pounds of which was collected this year. The food drive was designed to coincide with the end of the school year and support children who usually depend on schools' free breakfast and lunch programs. Departments within the hospital compete among themselves to collect the most food. This year's collection benefited Ocean County Hunger Relief, Food Bank of Monmouth and Ocean Counties and Morning Star Presbyterian Church of Bayville. Women and Heart Disease; Community Medical Center Raises Awareness of Heart Disease in Women As a Spirit of Women and Heart Caring designated hospital, CMC is uniquely poised to raise awareness of medical issues affecting women, including heart health. Heart disease is the No. 1 cause of death in the United States. It is also one of the most preventable. The hospital helped to ensure that women make their health a priority during American Heart Month by offering free blood pressure screenings and cholesterol and glucose testing as part of its Day of Dance 2014 celebration, a series of health education activities designed to reach women in all stages of life. A cardiologist offered a presentation on the importance of heart health, and CMC invited local dance groups to perform throughout the day to encourage people of all ages to dance and keep moving as a way to stay heart healthy.
