Our Lady of Lourdes Memorial Hospital, Inc. regularly and consistently monitors and enforces compliance with the conflict of interest policy in that any director, principal officer, or member of a committee with governing board delegated powers, who has a direct or indirect financial interest, must disclose the existence of the financial interest and be given the opportunity to disclose all material facts to the directors and members of the committees with governing board delegated powers considering the proposed transaction or arrangement. The remaining individuals on the governing board or committee meeting will decide if conflicts of interest exist. Each director, principal officer and member of a committee with governing board delegated powers annually signs a statement which affirms such person has received a copy of the conflict of interest policy, has read and understands the policy, has agreed to comply with the policy, and understands that the hospital is charitable and in order to maintain its federal tax exemption it must engage primarily in activities which accomplish its tax-exempt purpose.

In determining compensation of the organization's CEO, the process included a review and approval by independent persons, comparability data, and contemporaneous substantiation of the deliberation and decision. Community Benefit Report: Providing community benefit is an important part of our Mission. It represents a vital link to our community & neighbors. Our strength is in our history and Mission. Over the past year, Lourdes continued to work diligently to serve our community and deliver the Mission bestowed upon us almost 90 years ago. During this past year, many have been watching the creation of the Lourdes Vestal medical complex located at 3101 Shippers Road, in Vestal, behind Lowes. Services will open in 2014. The building and services have been designed with the current and future health care needs of our patients and the community in mind, for persons of all ages. Our goal has always been to be true to our mission in creating a positive patient and family experience, in a healing environment which is safe and provides quality care. The patient has been the focal point in all decisions which have been made around the services which will be offered, including facility design, location, and hours of operation. This has been an exciting opportunity to bring many services into one location; a benefit for patients whose primary care and specialty practices do not have on-site ancillary services, such as lab, x-ray, mammography or physical therapy. The Johnson City and Southside primary care practices which were temporarily relocated following the devastating flood of 2011 will be relocated to Lourdes Vestal along with internal medicine, dermatology, lab, radiology, occupational health, physical therapy, women's health, including screening mammography, diagnostic mammography, breast ultrasound , and a breast surgeon's practice. New services include a walk-in, bone densitometry, retail pharmacy and a community education room. The array of services offered in one location will allow our care team to provide integrated care that addresses community health priorities. We are especially excited to have on-site pharmacists and education programs that will support improving community health. We continue to take seriously our stewardship to provide access and coverage to quality health care services. In order to assist our community to prepare for the future, we increased the number of our financial counselors to eight, and ensured that all became Certified Applications Counselors. In this capacity they are able to help the community enroll in and understand coverage options available through the Affordable Care Act. This year access to primary and specialty care was expanded through the hiring of seventeen providers. We took many steps during this past year to address the mental health crisis in our community. As a result of these initiatives and others, Lourdes' Patient Financial Assistance Program enrolled 7,300 persons, up 12% from 2012. Beyond the walls of Lourdes, our associates extended our ministry into the community by serving on community boards, participating in community awareness events, providing supplies to elementary schools, participating in food drives, to over 650 families. I express my heart felt gratitude to them for living our Mission. As a leading health care provider, each year we provide millions of dollars in charity care & health education programs to our community. Lourdes is pleased to report to the community the services provided during 2013 which address the Community's Health Status and Health Priorities. Based on a comprehensive community health needs assessment conducted during 2013, Lourdes will be working with community agencies to address community health status around obesity prevention and the associated chronic diseases, falls prevention among the elderly and mental health and substance abuse. We look forward to continuing to partner with the community as we fulfill our call to provide Health Care that Works, Health Care that is Safe, and Health Care that leaves No One Behind. Our relationships inspire us and make our community a stronger, healthier place to live. We invite you to continue to participate in furthering the healing ministry to which we've been called. Priorities for the Year During 2013, Lourdes collaborated with community agencies under the direction of the New York State Health Department and County Health Departments to address public health issues and underlying causes and contributing factors to health status and chronic disease. This report to the Community demonstrates how Lourdes has worked over the last year to help the Greater Binghamton community in the following community health priority areas: Community Health Priorities * Access to Healthcare - Increase health care coverage - Increased access to behavioral / mental health and substance abuse - Increase access to prescription drug medication. * Preventing & Managing Chronic Disease, with a focus on decreasing obesity, diabetes, and reducing hospitalizations due to heart failure. In addition to these community priorities, Lourdes remains committed to addressing community needs in the areas of Cardiovascular Disease, Maternal/Child/Adolescent Health and the Frail & Elderly. Community and professional outreach and education initiatives were designed to address health priorities. During 2013, Lourdes also worked with local health departments and community agencies to identify community health priorities that will be addressed during 2014-2017 which are obesity prevention and the associated chronic diseases, falls prevention in the elderly and mental health and substance abuse. Commitment to Community Health Status Access to Health Care Increased Health Care Coverage - Lourdes made a significant investment in helping to provide healthcare access and coverage to help the uninsured. Lourdes increased the complement of financial counselors from six to eight during the year. All of the counselors completed Certified Application Counselor training and are certified by the New York State Department of Health. In this capacity counselors are assisting patients with enrollment in Affordable Care Act, ACA, plans, NYS Medicaid and the Lourdes Patient Financial Assistance program. The number of persons who are enrolled in the Lourdes Patient Financial Assistance Program was over 7,300, up 16% from the previous year. Over 2,300 persons were screened for healthcare eligibility with 96% being enrolled into a health coverage program. During the year, outbound calls were made to the uninsured populations to attempt to assist them with health care coverage options. The number of persons who were enrolled in the Lourdes Patient Financial Assistance Program was nearly 6,900, up almost 12% from the previous year. - Lourdes annual Diversity Fair focused on "The Divers World of Addictions". The intent around the fair was to increase awareness of the numerous forms of addiction that are rampant in our society and ways in which people can be helped. Increased Access to Primary Care, Prevention and Specialty Services. - Lourdes expanded access through the opening of a dermatology practice. - Lourdes assisted the community in ensuring access to primary and specialty care by employing seventeen providers specializing in primary care, endocrinology, and dermatology. Additionally Lourdes assisted with the recruitment of other specialties to the Broome County community to promote access to various specialists for the residents of our service area. - Lourdes Center for Oral Health provided Dental Services to 4,246 patients at our clinic site at 219 Front St, and 505 patients at our mobile clinic at 19 school-based sites (including Head Start). We also served 467 patients in the School-Based Dental Sealant Program and provided Oral Health outreach and education to 2,555 persons in our community. Increased access to behavioral & mental health, substance abuse, and Maternal, Child and Adolescent Health - Lourdes Youth Services touched the lives of many children and families during 2013 through a variety of programs. Lourdes Center for Mental Health (LCMH) served 850 clients in 2013. The Lourdes Student Assistance Program (SAP) and Alcohol and Drug Education Prevention Team (ADEPT) provided alcohol and drug abuse prevention services to 9,043 students and parents through a variety of evidenced based programs. ADEPT offered education to over 350 Lourdes associates and local community members on topics including: Adolescent Brain Development, Current Drug Trends, Gambling, Signs and Symptoms of Substance Abuse, Methamphetamine for Home Visitors and Children from Substance Abusing Parents. - Lourdes Youth Services provided services to at-risk adolescents through two programs in 2013. The Lourdes Mental Health Juvenile Justice Project (MHJJ) worked successfully to keep 98% of the youth served out of detention and out of home placement. This was accomplished by providing intensive counseling and case management as well as providing life skills enhancement groups for the participants.
