The person who was submitting the required 990 postcard, became ill and believed she had done so. The next person given this responsibility became confused as to which year she had to file the required 2011 return. When she reviewed the records on the filings she saw that a filing was made in the year 2011. It turn out to be for the year 2010, even though it was filed in the year of 2011.
